Overview

The Day Triage RN acts as a resource to patients/caregivers and field clinicians on clinical direction, issues, patient concerns, and problem-solving. Functions as a liaison between staff and physicians to facilitate coordination of care to Fairview Hospice patients. Utilizes excellent customer service skills and clinical expertise, protocols, and resources to direct or provide appropriate intervention.

This position will provide day triage coverage for Princeton and Chisago Lakes hospice teams and will travel between both office locations as needed.

Responsibilities/Job Description

Acts as a Liaison between staff and physicians to facilitate coordination of care to Fairview Home Care and Hospice clients.

  • Serves as a resource to the referral sources, staff, and physicians regarding Fairview Home Care and Hospice services.

Provides clinical support to field staff.

  • Communicates effectively and timely with interdisciplinary team members and patients.

  • Clinical resource for questions about diagnosis, procedures or equipment.

  • Faxes or calls MD with lab follow-up as requested by field staff.

  • Obtains and enters MD orders, communicates changes to patient and RN Case Manager.

Assists in the triaging of visits, putting patient safety first in all decisions.

  • Makes clinical decisions on issues relating to staffing.

  • Collaborates with colleagues to resolve patient issues or immediate concerns and completes all appropriate documentation. Reports issues to Clinical Manager or Director as needed.

Manages triage calls from patient &/or caregiver and, using clinical knowledge and expertise, resolves the issues on the phone or initiates skilled nursing visit as needed. Completes timely documentation and notification to Case Manager.

Qualifications

Required

  • One year recent RN experience in Home Care/Hospice Care

  • Computer skills: able to document effectively in EMR

  • Excellent customer service skills.

  • Licensed as a RN and eligible to practice in Minnesota

  • Current CPR certification (BLS for Health Care Providers)

Preferred

  • Bachelor's degree in Nursing

  • Two plus years of Home Care experience preferred.

  • Internal Fairview Home Care and Hospice experience preferred.

Additional Requirements:

  • Must have reliable transportation

  • Current Minnesota driver's license (or license that matches your permanent residence)

  • Must carry current and adequate automobile insurance
