Serves as a valued member of the departmental management team as the RN Coordinator on the floor to provide guidance to other RNs, LVNs, MAs and other clinical staff or administrative staff if performing duties for a clinic. Performs complex and varied administrative and clinical assignments. Serves as a personal contact for physicians, staff, patients and outside departments in order to address problems, issues or concerns that require attention. Assists in management of clinical unit operations. Serves as the primary competency validator/educator for the clinical staff. Able to provide telephone triage for same day appointments, and oversees a team of triage nurses. In addition, as an integral part of the multidisciplinary team, the Registered Nurse utilizes the nursing process to provide therapeutic care to patients and their families in the outpatient setting, when required. Ensures patient's right to privacy, safety, and confidentiality is maintained. Maintains a safe environment in accordance with standards, policies, and safety regulations. Ensures compliance with infection control policies. Complex, independent problem solving when overseeing a team of clinical floor staff and patient care coordinators with overlap at multiple practice sites or overseeing patient care if working at a clinic. Other duties as assigned.

In collaboration with the interdisciplinary team, the RN Coordinator provides care coordination services evaluating options and services required to meet an individual's health care needs to promote cost-effective, quality outcomes. Serves as a consultant to members of the health care team in the management of specific patient populations. The role integrates the functions of complex case management, utilization management, quality management, admission/discharge planning assessment, and coordination of outpatient care services, including transfers to an alternative level of care. As more complex medical treatment options emerge the RN Coordinator will look to eliminate gaps in the care provided, as well as needlessly duplicated treatment, all while controlling the cost of quality patient care. The RN Coordinator works with the interdisciplinary team to ensure any medical and/or mental health care needs are met by way of the clinical process of assessment, planning, implementation, and evaluation to the process of case management. Manage assigned caseloads of patients with Eating Disorder located at USC Student Health within Keck School of Medicine. Assesses patient's needs, develops, implements and reviews progress of service plans, and works with other community resources in meeting/achieving patients service needs. Coordinating and managing depression screening in medical services at USC Student Health. Works with the primary care providers and behavioral health consultant (BHC) to ensure adequate follow-up for students that screen positive for depression in the primary care setting. Educates patients regarding depression, monitors depressive symptoms and response to treatment using a validated instrument, and works closely with the primary care provider and BHC to revise the treatment plan when students are not improving and/or are unable to access appropriate follow-up care. Provides face-to-face meetings with patient and/or relevant staff, on a pre-determined frequency, reviews progress toward meeting service goals and updates or modifies the service plan as appropriate to ensure that the goals and objectives are being achieved. Provides on-going support and/or case management in accordance with the issues, needs and treatment strategies identified within the service plan in order to assist the students to achieve the stated goals and objectives. Participates in regular interdisciplinary treatment team meetings Accepts assigned cases following diagnostic intake by a licensed provider. Completes case management assessment on all assigned students. The RN Coordinator confers and reviews with clinicians, athletic trainers, and physical therapists to assess medical necessity and uses evidence-based criteria to refer patients for alternative care, level of care, intensity of service to support access to services. The RN coordinator facilitates timely case reviews to appropriate payor to ensure services are authorized/approved and documented. Clinical reviews and continued services authorizations will be documented in the electronic health record. Identifying patients 'insurance coverage or other sources of payment for services; Identifying and addressing patient risk factors and/or obstacles to care; Identifying patient needs, current services, and available resources, then connecting the patient to services and resources to meet established goals; Communicating the care preferences of patients, serving as their advocate, and verifying that interventions meet the patient's needs and treatment goals; Screening patients and/or population for healthcare needs; Developing a patient-focused case management plan; and Educating the patient/family/caregiver about the case management process and evaluating their understanding of the process. Some of the major duties of the Case Manager overlap into inpatient duties including: Concurrent review of all patients to validate that the appropriate patient status is assigned upon admission and prior to discharge; InterQual or MCG reviews are completed within 24 hours of admission; Observation patients are effectively care managed on a daily basis; and Facilitate throughput and timely discharges throughout inpatient level of care.

Essential Duties:

  • Follows policies and procedures: demonstrates ability to access online manuals and references

  • Serves as Lead RN and resource to clinical staff. Supervises at least two full-time staff or the equivalent, coordinates personnel actions, including, but not limited to performance evaluations, time-off requests, staff disciplinary actions, hiring, and terminating staff.

  • Serves as a personal contact for physicians, staff, patients and outside departments in order to address problems, issues or concerns that require attention.

  • Trains and competency validates all clinical staff to ensure compliance with hospital policies.

  • Acts as liaison between supervisor and his/her subordinates for communicating information, assignments, priorities and special requests. Monitors status of pending items and follows-up, as needed.

  • Ensures that exam rooms and treatment areas are supplied and maintained adequately by support personnel.

  • Promotes security by alerting appropriate personnel for safety, fire and other unsafe situations. Reports any unusual occurrences to supervisor. Responds to emergency situations using standard emergency protocols and procedures.

  • Participates in monitoring and evaluating the quality and appropriateness of patient care. Contributes objective data regarding performance of other clinical staff members, as requested.

  • Maintains a safe environment in accordance with standards, policies and safety regulations. Ensures compliance with infection control policies.

  • Under the direction of a physician and operating within the scope of practice, triages patient phone calls and provides guidance and direction to inquiries per protocols.

  • Transfers learned scientific knowledge into practical application in applying the nursing process

  • Integrates the functions of complex case management, utilization management, quality management, discharge planning assessment, and coordination of counseling, primary, and specialty care services, including transfers to an alternative level of care. As more complex medical treatment options emerge, will look to eliminate gaps in the care provided, as well as needlessly duplicated treatment, all while ensuring quality and coordinated patient care.

  • Leverage their clinical and social work experience to bring an understanding of the clinical process of assessment, planning, implementation, and evaluation to the process of case management.

  • Develops appropriate treatment/service plans, including discharge plans. Develops effective rapport with students.

  • Documents all service contacts on a timely basis including face-to-face interviews, collateral and networking contacts, correspondence and maintains the case records in accordance with agency and regulatory standards and requirements.

  • Maintains close communication with the medical and clinical staff for input regarding medication compliance, side effects of medication, and medication changes

  • Administers a validated instrumented for monitoring depression (i.e. PHQ-9 or similar) Monitors patients for changes in severity of depressive symptoms , co-morbid issues or diagnoses, medication side effects. Encourages adherence to treatment plan. Maintain appropriate professional boundaries with current students, family members, referrals sources and vendors.

  • Ability to take medical vitals (weight, BP, etc) as needed.

  • Documenting patients' case management plans and on-going activities Identifying patients 'insurance coverage or other sources of payment for services.

  • Identifying and addressing patient risk factors and/or obstacles to care. Communicating the care preferences of patients, serving as their advocate, and verifying that interventions meet the patient's needs and treatment goals. Developing a patient-focused case management plan.

  • Educate the patient/family/caregiver about the case management process and evaluating their understanding of the process.

  • ASSESSMENT: Appropriately completes nursing assessment and screening upon patient arrival at each appointment, in case of need.

  • DIAGNOSIS: Interprets assessment information and formulates nursing diagnosis

  • PLAN: Trains staff to maintain problem list, medication list and allergies list

  • INTERVENTION: Administers medications as ordered and completes re-assessments as indicated. Completes patient care procedures correctly per policy. Delegates tasks based on others' scope of practice and competency. Acts as patient advocate.

  • DOCUMENTATION: Documentation is complete and correct on all encounters. Ensures medication list is current and complete.

  • EDUCATION: Trains staff to provide patient and family education as needed based on diagnosis or treatment plan.

  • Evaluates nursing competencies for all licensed staff.

  • COMMUNICATION: Communicates effectively with all members of the healthcare team including the patient and family by:

  • Appropriate patient/family interactions, Effectively triages phone calls, Utilizing e-mail system, and Communicates effectively with members of the clinical team.

  • ORGANIZATION: Manages time effectively. Able to organize and prioritize responsibilities. Assists in overseeing daily clinic operational flow

  • OBSERVES REGULATORY AGENCY REQUIREMENTS: National Patient Safety Goals, Uses two patient identifiers, Does not use unapproved abbreviations

  • Uses procedural time out, Uses appropriate communication between providers, Completes medication reconciliation, Maintains audible alarms, Hazardous waste management, Biohazard, Medication, Sharps, HIPPA, OSHA requirements (ie. No food or drink in clinical areas), TJC (continuous survey readiness)

  • Critical values are written, read back and shared with appropriate provider, Dictated orders are written down, read back for validation, signed, timed and dated, Maintains a safe work environment for self and others.

  • PROFESSIONAL DEVELOPMENT: Participates in professional hospital/nursing activities. Assist in the development or updating of patient education materials.

  • Performs other related duties as assigned.

Required Qualifications:

  • Req Bachelor's degree Nursing Degree in Nursing (BSN)

  • Req 5 years In outpatient/inpatient clinical practice.

  • Req Experience in supervising clinicians.

  • Req Must have excellent communication skills, including the ability to speak, read and write English proficiently.

  • Req Knowledge of medical procedures and conditions and ability to understand, follow and successfully perform duties, including whole blood collection, and apheresis procedures, in accordance with administrative and clinical policies, regulations and procedures.

Preferred Qualifications:

  • Pref Experience in Urology Care.

Required Licenses/Certifications:

  • Req Registered Nurse - RN (CA Board of Registered Nursing)

  • Req Basic Life Support (BLS) Healthcare Provider from American Heart Association

  • Req Fire Life Safety Training (LA City) If no card upon hire, one must be obtained within 30 days of hire and maintained by renewal before expiration date. (Required within LA City only)

The annual base salary range for this position is $133,120.00 - $219,648.00.
