As an RN Coordinator, you will support our Pediatric Multidisciplinary Programs of Craniofacial and Infectious Disease at the Children's Health Center. You will function as an advanced practice nurse coordinating all clinical activities for pediatric patients with complex diseases in the outpatient setting. Responsibilities include, but are not limited to:

  • Deliver comprehensive case management, including nursing services, patient education, coordination of medical authorizations, referrals, and follow-up care

  • Evaluate and streamline patients' care regimens, developing and implementing diagnostic and therapeutic treatment plans

  • Oversee discharge planning and ensure smooth outpatient care coordination

  • Primary point of contact for cases requiring follow-up investigations with the LA County Department of Public Health

  • Serve as the Nurse Coordinator for assigned California Children's Services (CCS) Team Center Care clinics

Salary Range: $59.46-88.39 Hourly

Required:

  • Minimum of 1 year of pediatric clinical experience.

  • Bachelor of Science in Nursing (BSN).

  • Current valid California R.N. license.

  • BLS from the American Red Cross or American Heart Association.

  • Skill in data collection and analysis sufficient to evaluate and implement proper patient care and promote quality improvement efforts.

  • Demonstration of skill in executing assigned projects in an independent and responsive manner.

  • Ability to work in an independent leadership role within a team of healthcare professionals.

  • Ability to assess a patient's physical and behavioral status, using nursing framework.

  • Ability to coordinate patient care using a team approach.

  • Ability to recognize abnormal laboratory and clinical data on a patient and take appropriate nursing action.

  • Communication skills necessary for translating medical/technical material to patients and their families in a more simplified fashion so that it can be understood.

  • Ability to establish and maintain ongoing, cooperative working relationships with laypersons and professionals from a variety of disciplines and at various organizational levels.

  • Knowledge of professional nursing standards & procedures and ability to assess clinical situations and triage clinical matters appropriately by level of urgency.

Preferred:

  • PALS Certified.

  • Bilingual oral and written communication skills (English/Spanish).

  • California Children's Services (CCS) paneled.

  • Familiarity with the CCS and Medi-Cal programs.
