Conexio CareACT Registered NurseFounded in 1985 as a subsidiary of the organization then known as Church Home Foundation, Conexios original mission was to help older adults with lifelong histories of psychiatric hospitalization to transition into the community. Our mission expanded in 1988 to include homeless services and substance abuse treatment.Today, Conexio Care serves as one of Delawares largest nonprofits, collaborating with government, community, corporate, and other philanthropic partners to maximize services for our states most vulnerable citizens.The RN is responsible for dispensing any medications the clinic may utilize in an outpatient addiction recovery environment. This candidate provides basic medical care to clients while maintaining standards of professional nursing practice in accordance with Conexio Cares policy and procedures and as ordered by the Medical Director or Physician, Nurse Practitioner, or Physicians Assistant; responsible for thorough, accurate reporting and documentation of the clients symptoms, responses, and progress. This candidate will also share responsibilities such as maintaining dispensing equipment, managing medication inventory, and reporting client treatment information.Responsibilities include the following: * Facilitates health care service needs of individuals served and ensures that these service needs are addressed. * Competes all required medical assessments pursuant to best practices, contractual obligations, and licensure requirements. * Ensures that an individual's health care services adhere to the best practice, licensure requirements, and are consistent with the policies, procedures, and protocols of contractors and payers of services. * Delivers effective wellness and medical services in accordance with policies and procedures; state and federal regulations; and extant professional, ethical, and legal principles. * Maintains an effective therapeutic alliance with individuals served. * Participates in care coordination activities and meetings. * Completes all required documentation, audits, reviews, and reports in a timely manner relative current and ongoing health care service needs. * Communicates and coordinates with individuals receiving services, healthcare providers, family members or legal guardian regarding individuals health care needs. * Utilizes the required Electronic Health Record to record and document all health care encounters and interactions. * Abides by state and federal regulations, policies, and procedures related to confidentiality, patients rights (PM 46), and exposure and infection control. * Participates in performance improvement activities to evaluate and resolve problems related to risk management, utilization review, and quality assurance. * Provides after hours on-call support to program staff. * Reports to Direct Supervisor-Team Leader * Community Base Services * On Call ServiceExcellent Perks and Benefits: * Medical, Dental, and Vision benefits* 401k, Life & Disability Insurance * Generous Paid Time Off (PTO)* Paid Training & Career Advancement* A Great Team Environment * Competitive Wages * Wellness ProgramRequired Education and Qualifications: * Bachelors Degree in Nursing or equivalent/ or Associate degree. * Delaware Registered Nursing License in good standing or Compact License * Certification in First Aid and CPR* Mandt * Valid drivers license in good standing with at least 3 years minimum continuous driving experience with no serious traffic infractions. * Experience with electronic health records preferred * Excellent communication skills and organizational skillso Excellent ability to work with clients with substance abuse, co-occurring and/or mental health disorders*
