Description:
Oversees and coordinates care for specific specialty area for an assigned caseload. Facilitates appropriate care, assists patients and families in finding and negotiating that care. Education
Minimum: Graduate of an approved (NLN or AACN) accredited school of nursing.
Bachelors of Science in Nursing required.
Preferred: Specialty certification in specialty area preferred and/or required to obtain within 12 months of hire date. (See Addendum for position specific information and certification specific preferences)
Credentials/Licensure
Minimum: Current licensure as a Registered Nurse (RN) in the State of MI, in good standing.
Related Experience
Minimum: Three years in nursing experience
Preferred: Previous Case management experience

Computer Competency
Familiarity with standard desktop and windows based computer system, including email, e-learning, intranet and computer navigation. Ability to use other software required to perform essential functions.

Minimum: Ability to navigate a clinical area. Ability to work in a fast paced environment with multiple interruptions.
ESSENTIAL FUNCTIONS:
Completes a comprehensive health assessment of the patient's physical, psychological, social, environmental, financial and functional status. Includes assessment of past and present treatment course. Able to understand and develop goals, treatment plan and provider options.
Collaborates with other clinical teams and medical staff in performance improvement activities including assessment, planning, implementation and evaluation of clinical outcomes and processes for identified populations.
Serves as clinical expert related to populations served providing education and internal consultation to other staff.
Utilizes critical thinking and problem solving skills to analyze and synthesize all data and formulate an appropriate case management plan that addresses the patient's individual and family needs.
Monitors and modifies the plan of care through an interdisciplinary and collaborative process in conjunction with all parts of the health care team.
Communicates with the patient, family and other health team members both verbally and in writing.
Participates in the development and implementation of policies, procedures and quality improvement planning for the assigned caseload.
Establishes and promotes collaborative relationships with physicians and other members of the health care team. Develops and promotes collegial relationships with all customers.
Serves effectively as a patient/family advocate. Promotes team conferences for patients.
Serves effectively as an educator, promoting high quality care for assigned caseload.
Serves as a liaison, negotiator and facilitator between patients, other healthcare providers and third party payers to establish a supportive environment for the assigned patients.
Establishes goals and metrics for monitoring the quality of the care provided as measured against industry standards.
Performs other related duties as assigned.
Establishes and promotes collaborative relationships with members of the ostomy / wound care multidisciplinary team in order to develop plans that optimize the patient's success.
Responsible for coordinating all clinical aspects of the patients; including patient care, education, quality assurance, psychosocial and financial assessments.
Advocates on behalf of patients and families within the multidisciplinary team and the community.
Facilitates/Navigates the scheduling of tests and procedures.
Participates in the development and implementation of protocol and standards of care for the ostomy/wound care patient in collaboration with the multidisciplinary team.
Educates patients and staff about the various procedures, potential complications and the continuum of care.
Facilitates ostomy/wound care interdisciplinary quality meetings.
Ensures all ostomy/wound care intervention outcomes are monitored and addressed with variances.
Participates in team conferences, rounds, or meetings in relation to care of the ostomy/wound care patient or program.
Serves as a liaison between the inpatient and outpatient settings.
Detailed Responsibilities:
All patient treatment notes and communication to be entered into the electronic charting system.
Provide patient/family education to include:
Assessment of learning needs, abilities and readiness to learn
Documentation of teaching activities
Provision of instruction as appropriate, e.g. pain management, medication, diet, equipment, activities after discharge. Educate to the designated plan outlined in the physician orders.
Provision of appropriate educational materials, as available prior to discharge.
Document all care provided accurately & completely according to Hospital and Department Policy.
Adheres to all Patient Safety Initiatives and Infection Control Procedures as outlined in the hospital's safety manual. Appropriate use of Hospital resources, supplies and equipment.
Process and utilize all computerized patient care functions, e.g. ADT, diagnostic test results, etc.
