Job Title: Clinical Services Supervisor

Responsibilities:

  • Coordinate all clinical services provided to hospice patients, ensuring alignment with organizational mission and regulatory standards.

  • Oversee day-to-day clinical operations to maintain high-quality care and service delivery.

  • Report directly to the Program Director.

Qualifications:

  • Registered Nurse licensed in Connecticut.

  • Must have meet one of the following qualifications:

    • Master’s degree from an approved program by the National League for Nursing or the American Public Health Association with at least one year of full-time clinical experience in a home health agency or related community health program that includes home care of the sick.

    • Bachelor’s degree in nursing and a minimum of three years of full-time clinical experience in nursing, with at least one year in a home health agency or community health program involving home care of the sick.

    • Diploma or associate degree in nursing and at least three years of full-time or equivalent clinical experience within the past five years, including at least one year in a home health care agency or community health program providing home care of the sick. Must have certification as a community health nurse from the American Nurses’ Association or completion of at least six credits in community health nursing theory or health care management within the past two years from an accredited college, university program, or school of nursing.

  • Proficiency in interpreting and applying safety rules, operational instructions, and procedure manuals.

  • Effective public speaking, communication, and presentation skills.

  • Strong problem-solving abilities in diverse situations.

  • Familiarity with Medicare and Medicaid requirements in Connecticut.

  • Ability to relate to and support terminally ill patients and families.

  • Excellent organizational, interpersonal, and documentation skills.

  • Valid Connecticut driver’s license.

Physical Requirements:

  • Able to lift heavy objects and perform physical tasks related to patient care.

  • Capable of frequent movement and balance maintenance.

Working Conditions:

  • Subject to varying environmental conditions inside and outside patient homes.

Essential Functions:

  • Direct healthcare operations and ensure compliance with care standards.

  • Coordinate care plans and oversee interdisciplinary team meetings.

  • Supervise and support clinical staff, including hiring and training.

  • Implement quality improvement initiatives and ensure adherence to regulatory guidelines.

  • Maintain confidentiality and ethical standards in all patient interactions.

Additional Duties:

  • Perform other tasks as assigned by supervisor.
