Under general supervision and according to established policies and procedures, provides assistance in developing, implementing, and supporting activity to improve patients' care and safety, and to reduce morbidity and mortality.
Assists with work groups and processes development. Acts as an advisor for performance and quality improvement teams. Facilitates compliance with regulatory performance improvement standards.
Establishes rapport and working relationship with disciplines required to participate in formalized performance improvement programs. Contributes knowledge and assistance when needed.
Maintains knowledge of concepts and goals of performance improvement.
Collects data from various sources for Core Measures and other select performance studies. Submits data in a timely manner. Maintains high degree of accuracy evidenced by validation scores from CDAC.
Maintains compliance of Core Measure requirements, establishes performance improvement initiatives in response to Core Measures data. Educates hospital staff and Medical Staff on Core Measures, changes in requirements, rates of compliance and outliers.
Maintains in-depth understanding of current vendor (Thomson-Rueters) software program. Is able to import data, exchange data and extract reports. Communicates regularly with vendor, is able to trouble-shoot and help resolve problems with data exchange. Serves in advisory capacity re: vendor contracts.
Maintains knowledge of current clinical practice.
Facilitates coordination of performance improvement measurement using performance improvement tools both statistical and non-statistical and reports from vendor. Leads select project teams. Utilizes DMAIC process to facilitate process changes. Incorporating LEAN principles as applicable.
Maintains working knowledge of computerized systems and programs including, but not limited to Midas, MyCare, HPF, Sunrise Analytics and Star.
Performs other duties as assigned.
Bachelor's degree in nursing or related field
High level of interpersonal and customer service skills in order to consult and work effectively and efficiently with employees, surveyors, physicians and other customers.
Leadership ability to motivate and direct professionals to accomplish meaningful performance improvement directives.
Must have basic knowledge of the concepts of performance improvement, third party payers, and other external review and accreditation agencies.
Excellent organizational and project management skills to perform duties and meet deadlines.
Analytical ability to formulate programs and studies to enhance patient care and identify barriers to those objectives.
Computer skills and ability to proficiently input/retrieve data; Must have working knowledge of Microsoft Office.
Must have good math skills as well as skills in research and statistical fundamentals.
Physical ability to move from one office/work area to another, occasionally lifting and/or moving binders, files, etc. Ability to bend, and reach for stored records or files.
