Job Summary:# The Clinical Nurse Educator provides nursing education to the Special Care Nursery staff to maintain standards of care related to newborns and their families.# Serves as educator, facilitator, change agent, consultant, researcher and leader.# Facilitates the orientation of new employees; Participates in Quality initiatives related to the Special Care Nursery; Participates in hospital committees and orientation as needed.# Works collaboratively with unit management to ensure program development, implementation and evaluation. Other duties may be assigned as necessary. * Accurately completes required start of care documentation within 24 hours. * Is proficient in HCHB processes. * Develops and implements the patient care plan in cooperation with other Agency professionals and paraprofessionals who will be providing care to the patient. Identifies additional disciplines/care programs for the patient and implements such using such guidelines. * Observes signs and symptoms and reports, to the physician, reactions to treatments including drugs and changes in the patient's physical and emotional condition. Obtains, as needed, physician orders to adjust the plan of care. Communicates changes to the plan of care to the nursing supervisor, case manager and other members of the Hospice team. * Ability to profile/reconcile medications in conjunction with pharmacy vendors and physician. Medication reconciliation and follow up if discrepancies * Order DME equipment specific to patient needs. * Has an understanding of Medicare disease specific criteria, hospice level of care and hospice benefit. * Has an understanding of the needs in the various settings we service (GIP, routine visit, respite, continuous car, hospital SNF ALF and home patients) * Teaches, supervises, and counsels the patient and family members regarding the nursing care needs and initiates preventive and rehabilitative nursing procedures as appropriate for the patient's care and safety. * Assesses and assigns Hospice Aides as appropriate by providing a written plan of care and specific duties the Aide is to perform for each individual patient. * Communicates with supervisor and, as indicated to other members of the Hospice team, concerning problems of the patient(s) on a timely basis and communicates information using current process and technology available to the organization. * Attends and participates in office staff meetings, patient care conferences, record audits, utilization review, and performance improvement, in services, workshops, and seminars. * Keeps up-to-date on reimbursement criteria and charting needs for all patients. * Strives to improve nursing care through continuing education, attendance at workshops and conferences, active participation in professional and related organizations, and individual research and readings. * Adheres to the Agency's personnel policies and Compliance Plan. * Displays a high degree of courtesy, tact, and knowledge of services provided by the agency in all contact with staff, patients, and visitors. * Respects patient rights, and property. * Must call in report daily to Supervisor or scheduler. * Able to complete 3 (three) SOC daily and if SOC's are not available, must do visits as assigned by the Supervisor. * Contacts the Nurse Supervisor to inform him/her of alternate schedule for patient visits during vacation, sick days, or personal days. Immediately reports any schedule change or inability to complete assignment as scheduled. * Accepts changes/additions to schedule during staffing difficulties when necessary. * Organizes daily visit schedule to minimize travel time and mileage. You will be expected to travel where business needs are located in order to reach your weekly productivity. * Untimely or poor documentation is subject to disciplinary action, up to and including termination. * Must travel to where business needs are located in order to reach productivity.
