Responsible for collaborating with healthcare provider team, community outpatient treatment providers, referral sources and utilizing resources to create positive outcomes for patient population served and facilitate continuity of care. Ensure smooth transitions from care setting to community resources. Identify and assist with removing barriers to restoration of wellness. Coordinate care with a special emphasis on support services.
Assess patient assistance needs based on diagnosis. Establish a follow up plan including appropriate resources to help patient and significant others provide support system vital to impact patient outcomes. Consult with unit associates regarding resource utilization, care planning and evidence based practice.
Acts as clinical resource, patient advocate, and consultant to assist all patients, including the underinsured/uninsured patient with care coordination. Facilitate recovery and rehabilitation planning before, during, and after treatment, i.e., pre and post by assisting patients with navigating the continuum of care. Provides information and education to patient and facilitate information, gathering, sharing, and analysis to assist patients with making informed decisions. Coordinate and develop educational materials for patients, families and staff.
Evaluate and make recommendations to improve the quality of patient outcomes and services provided. Utilize analytics to determine area(s) of improvement and initiate positive solutions.
Education, Credentials, Licenses:
BSN
Current registration to practice nursing in the state where work is being performed.
Obtains required CEUs including all state required courses
NOTE: Must meet requirements below:
(within 6 months of employment)
Must have a legal and valid drivers license
Must pass a criminal background check
Must pass MVR check
Must be insurable
Must be considered acceptable as a routine driver by healthcare systems business auto insurance carrier.
Specialized Knowledge:
Extensive knowledge of nursing standards and practice in the clinical specialty area
Kind and Length of Experience:
3 years of experience in Nursing or related field. Demonstrates ability to work autonomously and can provide direction to nurses, physicians, and ancillary associates.
