Job Duties

Under the supervision of Care Coordinator Manager/Supervisor, Primary Care performs tasks and patient contact to assist in the coordination of patient primary care services. Services include obtaining and running reports to assist with review and resolution of care gaps.

Essential Functions

  • Reviews monthly Care Gap data from the Managed Care Department for Family Medicine and General Medicine clinics.

  • Performs Transitional Care Management, (TCM) non face-to- face patient visits per policy and procedure

  • Contacts insurance Case Managers under the direction of the Care Coordinator Manager/Supervisor, and implements plans to follow up with patients, or to include phone calls, video discussions to assist in preventing or reducing ED visits.

  • Reviews Healthy Planet Performance of each clinic monthly and identifies measures that require improvement, and communicates to Care Coordinator Manager/Supervisor.

  • Maintain a positive customer service oriented disposition toward all clients and co-workers. Provide excellent customer service with internal and external customers.

  • Prepares and maintains all clerical aspects of the medical records through established guidelines.

  • Performs all other duties as assigned by management within job scope.

Temperament

Must be able to perform under stress when confronted with emergency, critical, or unusual situations. Must be capable of dealing with periodic cyclical workload pressures and levels of responsibility. Required to make independent judgments without supervision. Must be able to make generalizations, evaluations, or decisions based on sensory or judgmental criteria. Must have the adaptability to perform a variety of duties, often changing from one task to another of a different nature without loss of efficiency or composure. Requires the ability to work with people beyond giving and receiving instructions.

Regulations/Policies

  • Knowledge of regulatory agency standards (e.g. Joint Commission, HRS).

Training/Support

  • Demonstrated Clinical skill in area of ambulatory care and/or particular specialization (e.g., Pediatrics, ENT).

Teamwork

  • Ability to work in a team environment.

Experience Requirements

  • 1 year experience on Ambulatory Primary Care setting
  • Required
  • 2 years experience on Ambulatory Primary Care setting
  • Preferred

Education Requirements

  • Associate Degree Nursing
  • Required
  • Bachelor Degree Nursing
  • Preferred

Certification/Licensure Requirements

  • RN Florida Licensure required at time of hire.

Perform quality assessments for physicians, facility and ancillary providers taking into account all available information internal and external to the organization. Collaborates with internal staff and network management to define potential quality improvement activities within the corporation, at the individual or group practitioner level; facility level or network system level and could potentially involve a third party in the collaboration. Leads the full spectrum of various quality improvement initiatives to include collaboration with key stakeholders (both internal and external) for design, implementation, monitoring and relationship management. Essential Job Functions Provide leadership where an opportunity for quality improvement activities is determined with network representatives and/or other key stakeholders to exist. Design, implement and monitor the initiatives. These opportunities could potentially involve internal personnel, individual or a group of physicians, a hospital or hospital system, ancillary provider or other delivery system entity and has the potential to involve a third party. Perform clinical quality assessment analysis to include all available information that comes from internal and external sources to include claims and other internal data, customer inquiries, peer review findings, publicly available data, industry information and others and provider findings to key stakeholders. Perform medical chart reviews where necessary to support improvement and compliance related activities. Manage the relationship with all involved parties. This is not a remote position Minimum Job Requirements Bachelor's degree (or additional relevant experience in lieu of degree) Must have a Florida Registered Nurse license 4 or more years' experience in health care and/or insurance industry Demonstrate high degree of initiative, able to work and follow through on assignments without close supervision Proven strong project management skills Superior written and verbal communication skills needed as well as strong interpersonal, organization and problem solving skills.
