SUMMARY

Under the direction of the Nurse Manager and Director, the RN communicates and collaborates with physicians and other members of the health care team to provide consistent quality care for in-patients and out-patients with wound, ostomy, and HBO needs/conditions. Working with a high degree of autonomy, the RN accepts responsibility for patients as a case manager to plan, implement and evaluate care for chronic wound, ostomy and hyperbaric patients.

The RN works within the framework of the Nurse Practice Act and follows all department and hospital policies and procedures. He/She complies with all hospital standards and nursing clinical practice guidelines while performing job duties and responsibilities.

Primary Responsibilities

1.Assessment- Assumes responsibility for an assigned group of patients. Is accountable for an assessment of the patient's basic needs.

Enables the care team to make appropriate decisions to develop, implement and evaluate the plan of care. This assessment will include biophysical needs, psychosocial needs, enviommental/safety needs, self-care needs, educational needs, discharge planning needs.

2.Clinical Pathways-Demonstrates knowledge of and understanding of clinical pathways for wound, ostomy and hyperbaric oxygen therapy.

3.Plan of Care- Prepares, implements and evaulates a plan of care in collaboration with the clinic/in-patient physician, the patient and authorized family members. Establishes goals, outcome criteria and target dates. Updates this plan of care to reflect changes in the patient's condition and response to therapies and/or education.

4.Documentation-Documentation of assessments and interventions is accurate, complete and timely.

5.Wound Competence -Demonstrates competence in all wound treatment modalities and interventions according to the Nurse Practice Act, clinical pathways, and department policies and procedures.

6.Ostomy Competence(If applicable)-Demonstrates competence in all ostomy treatment modalities and interventions according to the Nurse Practice Act, clinical pathways and department policies and procedures.

7.HBOT Competence (If Applicable)- Demonstrates competence in hyperbaric treatment modalities and interventions according to the Nurse Practice Act, clinical pathways, and department policies and procedures.

8.Urinary Catheter Competence-Demonstrates competence in all urinary cather treatment modalities and interventions according to the Nurse Practice Act, clinical pathways, and department policies and procedures.

9.Diagnostic Data Management-Integrates and documents diagnostic data into health assessment notifying physicians of results in a timely manner.



  1. Patient/Family Education-Provides patient and family education on common topics related to wound, ostomy, urinary catheter and hyperbaric oxygen treatments.

  2. Scribing-Demonstrates competence in scribing clinic procedures, physician orders, etc for out-patient care in the wound clinic.

  3. Key Success Factors-Exhibits key success factors which include but are not limited to: reliable attendance, punctuality and professional appearance.

  4. Annual Requirements-Performs timely completion of: Annual health requirements, online annual update, ethics training, CPR and professional certification/licensure.

  5. Performs other duties as assigned.


TRAINING AND EXPERIENCE

MANDATORY:

1.Possession of valid Registered Nurse License issued by the state of California.

2.Valid BLS Card for health care providers.

PREFERRED:

1.Wound Care experience.

2.Wound Care certification.

PHYSICAL REQUIREMENTS

1.Must be able to safely lift or turn patients using proper body mechanics or with assistance. Work requires walking, standing, sitting, lifting, bending, and pulling. Must be able to ambulate throughout the hospital as well as push patients in wheelchairs and/or gurneys.

2.Must have the dexterity, coordination, visual and auditory acuity to perform all duties.

Pay range: $ $45.08 - $69.86 an hour

Individual wages are determined based upon a number of factors including, but not limited to, an individual's qualifications and experience
