Our established community in St. Cloud, MN is accepting applications for an RN Case Manager to join our team!
Position is full-time. Working primarily daytime hours with flexibility to meet the changing needs of our residents and staff during evening and weekend hours. Position includes a shared on-call responsibility.
POSITION SUMMARY: Responsible for implementing and enforcing the nursing process in accordance with our Medicare comprehensive license and company policies. Responsibilities include but are not limited to:
Responsible for assisting in orientation, onboarding and training nursing staff for their ongoing success
Must be able to communicate effectively with all levels of the organization, including resident/family, and physicians
Responsible for resident outcomes, ensuring appropriate care, internal audits, assessment of resident needs and modifying the care plan as needed
Provide direct nursing services to clients and through the coordination of all disciplines involved in client care, as assigned by the Manager
Assist each resident to reach and maintain his or her highest level of performance and independence
Responsible for resident outcomes, ensuring appropriate care, internal audits, assessment of resident needs, and modifying the care plan and/or Residential Services (Customized Living) Tool as needed
Current Licensure as a Registered Nurse in the State of Minnesota
Must be professional, and mature to work effectively and respectfully with staff, managers, executives and others with a variety of working styles to achieve accountability and results
Bachelor's Degree in Nursing is preferred, but not necessary
Technical nursing skills preferred
PHYSICAL ESSENTIAL FUNCTIONS:
Ability to sit, stand, speak, and walk for long periods of time
Ability to bend and stoop using good body mechanics
Frequent lifting, carrying, pushing and pulling up to 30 lbs.
Occasional lifting, carrying, pushing and pulling 75-100 lbs.
Some exposure to infectious waste, toxic chemicals, needles/body fluids, and mechanical hazards
The successful candidate must able to delegate, have a sound judgement with a collaborative style that fosters teamwork and integrity. Must have a passion for excellent customer service and commitment to exceptional quality.
