PositionSummary:

Coordinates patient care between patients, caregivers, and themultidisciplinary team; collects and evaluates information to facilitate careby assuring that admissions, continued stays, and ancillary services aremedically necessary, cost-effective, and providing the appropriate setting toqualify for reimbursement. The case manager intervenes when necessary toassure that case is care is coordinated and timely.

PositionResponsibilities:
1.Documents on the day ofadmission, or within the first twenty four hours, a clinical assessment andanticipated discharge plan that addresses use of the full continuum of care
2.Provides plan of careupdates to providers inside and outside of the hospital, working with patientsand caregivers
3.Continually assesses andmodifies the plan of care based on clinical need and patient preference
4.Manages each patient'splan of care and throughout the stay
5.Leads interdisciplinaryrounds to coordinate care with the care team, and to address changes in thedischarge plan
6.Consults withappropriate physicians on plan of care and discharge
7.Remains current ofMedicare and Medicaid laws and guidelines, Medicare Conditions ofParticipation, managed care contracts, and other legal and regulatoryrequirements as they relate to health care case management services. Assures patients are informed of rights and responsibilities asparticipants in these programs.

Dependent on the facility the following responsibilities may berequired:
8.Identify and executedischarge planning needs determined through patient/significant other contact,chart reviews, health care team rounds and clinical pathways and protocols. Thecontinuum of care may extend from time of admission through the Rehabilitationphase.
9.Liaise with physicians,patient benefits, and third party payers to identify, reconcile and executenecessary actions for services rendered, ensuring appropriate services andreimbursement.

Education:

Bachelor'sDegree - Nursing OR ADN and 5 years of clinical experience

Licensure/Certification:

RegisteredNurse (RN)

Minimum Experience:

Three (3) years of clinical nursingexperience in the acute care or medical setting OR ADN and 5 years of clinicalexperience

MinimumKnowledge, Skills, and Abilities:

Clinical practice with experience acquired in assigned specialtyrequired

Computer skills required; must be able to document in medicalrecords

Exercises knowledgeable, independent judgment on routine andnon-routine matters

PositionSummary: Responsible for providing routine and complex care to varioustypes of patients seen by this service line provided in accordance with currentstandards and care and as established accrediting bodies and regulations suchas by Society of Oncology Nurses. Responsible for total patient care, includingvenipuncture, administration of Chemotherapy, invasive and non-invasivemonitoring, quality control of equipment, wound care, accurate and timelyrecord keeping, and specialized care as needed. Manages patients' plan of care collaborating with the staff and otherdisciplines within the unit (i.e., completes/Coordinates patient assessment,Develops/monitors outcomes based on plan of care, Coordinates and Implementsplan of care, evaluates outcomes and monitors variances). Provides hands-onexpert care to patients and accountable for managing patient care in order tomeet optimal outcomes. Responsible for cross training to provide delivery ofacute and chronic care within the service line, and admission assessment inaccordance with current standards of care. PositionResponsibilities: 1. The RN delegates and collaborates with other staff members 2. Collaborates and communicates with physicians to ensure qualitycare 3. Develops, Utilizes, and models effective time management skills 4. Plans for appropriate patient populations, and Manages plan ofcare with interdisciplinary team through ongoing collaboration and use of careplans 5. Participates in development of collaborative care 6. Coordinates discharge and outpatient plan of care withpatient's family 7. Collects data to utilize and analyze outcome-based caremanagement variance reports to evaluate patient progress 8. Communicates with all team members regarding therapeuticinterventions including patient/family/significant other 9. Sets patient care priorities 10. Responsible for total patient care, including venipunctures,administration of chemotherapy, invasive and non-invasive monitoring, and anyother patient care as prescribed. 11. Delegates appropriate tasks to licensed staff, monitoring andmaintaining accountability for the quality of care 12. Utilizes interdependent team decisions making to ensurequality of care 13. Develops and coordinates the education plan appropriate to thepatient/significant other family learning style. Communicates plan topatient/significant other/family and or interdisciplinary care team members 14. Manages resources, which promote cost effectiveness, efficientoperation of unit, and positive outcomes 15. Performs comprehensive patient assessments, anticipatingpotential care problems and prioritizing care needs in order to effectivelyprovide optimum care 16. In collaboration with staff members, provides bedside patientcare, implementing the interventions identified in the plan of care 17. Document patient's plan of care and interventions, accordingto policy 18. Initiates and communicates change in patient care deliverybased on continual evaluation of progress according to outcome-based care plan 19. Provides input into employee feedback process for the othermembers of the staff including the Medical Director 20. Initiates collaboration with physician and interdisciplinaryteam members by effective communication of the pertinent patient information toinsure continuity of care 21. Participates in education and training for the staff; coachesand provides feedback on an ongoing basis to all team members; Serves as amentor for the staff 22. Make recommendations to improve patient care delivery process 23. Acts as a clinical resource to manage difficult patient/familysituations 24. Maintains and demonstrates competency in BLS (Basic LifeSupport), infection control, safety and all unit-required skill review 25. Utilizes interventions substantiated by research asappropriate to the practice environment Education: Associate'sDegree - Nursing degree from an accredited nursing school Licensure/Certification: RegisteredNurse (RN) BLS(Basic Life Support) Certification.
