Job Summary: Facilitates processes on a daily basis to expedite patient care analyzing variations from predicted utilization.
Coordinates input from all health professionals, conducts assessment of patient and family needs and formulates a documented discharge plan assuring continuity of care. Facilitates appropriate use of hospital resources by reviewing patients admitted to the hospital. Facilitates appropriate communication to insurance company for post-acute care services.
Minimum Requirements: Registered Nurse; BSN.# Will consider Associate degree RN on a case by case basis. Interpersonal skills necessary to negotiate with families, patients, physicians, and third party payors for optimum plan.# Excellent communication (oral and written) skills required to ensure patient, family and team satisfaction.# Ability to work independently, setting priorities to coordinate care plans efficiently under constraints of managed care.# Knowledge of Community Agencies required to provide patients and families with appropriate choices and options for post hospital care or prevention of admission.
Preferred Qualifications: 2 years experience in a health care related setting.# MSN preferred #Two years previous Utilization Review, Discharge Planning, Case Management experience; Case Management Certification. Licensure and Certifications: RN licensure # #LI-ML1 HTFSJ
