NURSE CASE MANAGER-Infectious Disease - Grant funded.

The RN Case Manager for Infectious Disease supports the provision of coordinated medical and integrated behavioral health services to patients within the Harm Reduction Program working with patients with HIV, TB (active and latent), Hepatitis B and Hepatitis C Virus (HCV), Sexually Transmitted Infections (STIs: Chlamydia, Gonorrhea, and Syphilis) and those who access Syringe Services.

The successful applicant will offer:

  • Graduate of an accredited nursing program.
  • RN with current Mass. Licensure
  • Case Management experience preferred.
  • Bilingual in Spanish/Khmer/Russian beneficial
  • A desire to work as a collaborative member of a multi-disciplinary team.
