RN Case Manager, Hospice

A Day in the Life of a Case Manager Nurse for our Hospice team:

• Travels to complete visits for each assignment

• Obtain, implement, perform, and maintain orders as received by physicians

• Initiates appropriate preventive and rehabilitative nursing procedures

• Teach patients and families or caregivers self-care techniques as appropriate

• Instruct, supervise, and evaluate home care aides as applicable.

• Instruct, supervise, and evaluate the visit nurses as applicable.

The pay range for this position is $33.15 - $41.44 / hour. Starting pay rate will be based on years of experience.
