Educational Requirements

Currently licensed as a Registered Nurse (RN) in the state and in good standing with the Board in which he/she will practice.

Position Requirements

A minimum of 6 months nursing experience within the past 3 years unless state regulations differ. Travel is necessary on a daily basis.

Position Information: The Home Health Care Coordinator is a Registered Nurse who is assigned to WakeMed hospitals, to assist with facilitating the plan of care for an inpatient episode and timely discharge to Home Health by using a collaborative interdisciplinary approach. Works in collaboration with the hospital discharge team to anticipate the needs of the patient and the services necessary to meet those needs. The Care Coordinator works collaboratively with the case management department and other departments, in assessing patient needs and developing the plan of care, once the patient has chosen WakeMed Home Health. The Care Coordinator will also act as an advocate for the patient, physician and hospital relative to compliance with federal, state and third party utilization management regulatory requirements pertaining to home health.
