Qualifications

A minimum of 6 months nursing experience within the past 3 years unless state regulations differ. Travel is necessary on a daily basis.

Educational Requirements

Currently licensed as a Registered Nurse (RN) in the state and in good standing with the Board in which he/she will practice.

JOB SUMMARY Manages the day to day operations of the Case Management Department department including personnel, financial, performance improvement and program management ESSENTIAL FUNCTIONS: * Oversees and directs employees in the Case Management department; directs daily operations to achieve effective utilization of personnel resources consistent with clients' needs; participates in interviews, recommends for hire, supervises and recommends discipline, including termination in accordance with Human Resources policies and procedures; completes performance evaluation process and provides input for employees; conducts regularly scheduled departmental meetings and one-on-one supervision meetings * Monitors functionality, safety and quality of programming; monitors employee performance to ensure high quality services are provided to clients; provides training, coaching and guidance to employees as needed to address areas of performance improvement; identifies and implements best practices; commits to continuous improvement and updates policies and procedures as needed; monitors client satisfaction and responds to client complaints in a timely manner. Is knowledgeable of length of stay and case mix index and ways of improving, Able to accept multiple task from a variety of sources and work on these initiatives to produce timely and favorable outcomes. * Manages and oversees employees in area of daily operations consistent with productivity and budget goals, policies and procedures of the organization and regulatory requirements; provides support and assistance to employees as needed; attends management and staff meetings * Identifies and achieves productivity goals and important quality metrics such as LOS and observation; readmission works collaboratively with the Director and other staff to provide actionable data for improvement. Actively participates on committees, task forces and other work groups. Researches and seeks opportunities for improvement in the delivery process, or specific patient care problems. * Works collaboratively and develops a relationship with healthcare faciities within the region, and case management professionals to improve care provided to our patients. Understands payer principles, terminology of managed care and the goals of manged care industry.
