The Registered Nurse Case Manager is responsible for assessing, defining and supervising the care coordination for all private duty clients. The Case Manager assures all clinical needs of our clients are met and works as a part of a team that maintains a close relationship with our clients and their families. (RN) - Private Duty Nurse Case Manager Job Duties include:

Meets with potential clients to assess client needs and work with them and their family to develop a plan of care to be followed by nursing aide staff. Regularly reevaluate the client’s needs and works with the family and the Branch Manager to identify and provide the services needed to allow our clients to thrive in a home setting Counsel the client and family on care related topics to assure client is receiving appropriate care. Supervise, educate and coach nursing aide staff.

Provide new staff orientation and training Be involved in day to day management of client needs working closely as a team with the Branch Manager and the Client Care Coordinator to maximize client satisfaction. Involved in community marketing activities with the Marketing Liaison to expand awareness of our clinical capabilities and establish a personal relationship with our referral sources. Demonstrated ability to supervise and direct staff.
