We currently have an opportunity for a dedicated and experienced Registered Nurse Case Manager looking to make a significant impact in the home healthcare sector. This full-time role is based in West Cuyahoga County, OH, covering regions including Parma, Middleburg, Brooklyn, Brookpark, Fairview Park, and Westpark. Key Requirements:

• An Associate's degree or higher

• Home Health Care and Medical-Surgical (Med-Surg) experience of over 1 year

• An active Registered Nurse (RN) certification/license valid in Ohio (OH)

• A valid Basic Life Support (AHA BLS) certification/license from the American Heart Association

• Over 1 year of experience, preferably within Burn Unit/Wound Clinic or Wound Care Preferred Qualifications: - Bilingual capability, particularly in Spanish

• Specialized certifications, if any Responsibilities:

• Deliver comprehensive nursing care, both directly and indirectly, to a pre-defined set of patients belonging to diverse age and diagnostic groups

• Conduct thorough data collection, planning, evaluation, and documentation of nursing care using established nursing methodologies

• Demonstrate competency in prevention, therapeutic, and rehabilitative nursing care

• Develop and maintain open lines of communication and collaboration with healthcare providers, patients, and their families

• Ensure patient comfort and safety, responding to life-saving situations as per established nursing protocols

• Regularly observe patients and report significant conditions and responses to treatments to the supervising authority EMR: Home Care Home Base Equipment Provided: Tablet, Phone Schedule: 8:30am-4:30pm, includes a mandatory 1 weekend per month Benefits:
