We are searching for a Care Coordinatore/Case Manager to join our Case Management team with primary responsibility and authority for evaluating each observation patient for inpatient intensity of services per InterQual criteria to include justification for admission, evaluation of care for appropriateness for continued stay, appropriateness of level of care ordered, and compliance with the delivery of health care services.
This is a full-time, day shift position with rotating weekend coverage.
A BSN is required in order to be considered for this opportunity.
Accountability for follow-up notification to ensure timely and ongoing authorizations when required for observation patients.
Responsible for the MIDAS Progression of Care documentation and case reviews every 4-6 hours and strong physician communication and collaboration via phone or in person several times per day in order to tightly manage patients in observation status.
Responsible for elevation to higher level of expertise when criteria not met, documentation not clear, status orders or level of care ordered is not supported.
Responsible for observation data tracking, trending and reporting.
To become the observation subject matter expert and will help educate other disciplines as needed.

Education: Registered Nurse required.
BLS certification required
Registered Nursing license with the state of Georgia is required
Certification in Case Management, preferred
Two years nursing experience in acute care setting, required
Case Management experience, preferred
