Job Title: Registered Nurse - "Wound Care Experienced"

Position: Per Diem hours

Job Summary

Responsible for diagnosing responses to actual or potential health problems and for providing basic emergent, supportive, and restorative care through use of the nursing process. Demonstrates knowledge and skills necessary to provide care appropriate to the age of the patients served in assigned responsibility.


  • Assesses the patient's physical, psychosocial, emotional, cultural, spiritual and discharge needs

  • Utilizes effective oral and written communication skills to include completeness and accuracy of documentation

  • Demonstrates accurate technical skill and operational knowledge of patient care equipment and procedure

  • Evaluates the plan of care and reformulates the goals as necessary

  • Develops and implements a plan of care for each patient

  • Recognizes unsafe and life-threatening situations and intervenes appropriately



  • Graduate of an accredited school of nursing required

  • Currently licensed as a Registered Nurse in the state of Connecticut required

  • Current AHA Healthcare Provider CPR certification or equivalent within three months of hire required

  • Graduate Nurse: primary source verification of successful completion of a nursing program directly from the nursing education program

The RN Care Coordinator uses best practice concepts to ensure appropriate service utilization, care coordination, and a positive person-centered experience for the patient and caregivers. The RN Care Coordinator collaborates with multidisciplinary patient care teams, internally and externally, to achieve high quality, cost-effective outcomes. ESSENTIAL RESPONSIBILITIES: * Initial review and evaluation of home health admission data and documentation for clinical appropriateness and completeness. * Facilitates coordinated and collaborative care plans with all involved providers. * Maintains utilization statistics in line with national and regional best practices to optimize Home Health and Hospice Compare scores. * Facilitate timely discharges and transfers based on individual needs and care requirements. * Coaches clinical care team based on review of clinical records in collaboration with the clinical manager. * Monitor care plan for cost-effective use of resources * Collaborate with insurance providers and care team to facilitate necessary patient services, including assistance with insurance authorization denials. * Partners with QA RN to perform clinical audits and ensures timely corrections and plan of care modifications by the admitting/assessing clinician. * Maintains and applies knowledge of and compliance with Federal, State, accrediting bodies, and agency policies and procedures. * Provides patient triage and routine care reviews telephonically using the electronic medical record, communicating with home health staff, providers, physicians, and patients. * Performs home health and hospice visits as needed. QUALIFICATIONS: Education: Graduate from a baccalaureate degree program accredited by National League of Nursing, diploma, or associate degree in nursing required. Valid Connecticut driver's license and proof of current insurance coverage required. Experience: Current license to practice as a registered nurse in the State of Connecticut required.
