Responsibilities

Oversees the day-to-day delivery of health services provided by the community.

Assesses the health, functional and psycho-social status of new residents and participates in the development of personal service plans. Coordinates healthcare delivery for each community to determine appropriate care through management of clinical services and assessment processes. Participates in personal service plan reviews with the management team, communities and residents' families. Regulates the medication and health-related supply distribution process, including appropriate documentation, ordering, storage, security, destruction and pharmacy consultations. Ensures the proper and timely documentation of health care according to Brookdale policies and procedures and in accordance with federal and state requirements. Delegates nursing tasks to direct care staff in compliance with the Nurse Practice Act in areas of basic health care and proper documentation. Delivers health-related in-service and medication management training. Assists Executive Director in communicating with family members regarding the changing health care needs of residents. Consults with health care specialists to identify necessary ancillary services and support. Collaborates with Executive Management to implement company policies and procedures to guide the delivery of health care services to communities. Participates in company quality improvement activities and quality assurance surveys.

Qualifications

Education and ExperienceEducation as required to obtain state RN license and a minimum of 2 years nursing experience.

Certifications, Licenses, and Other Special RequirementsCurrent nurse (RN) license, issued by appropriate state licensing agency. Must have a valid driver's license and access to a private vehicle for business use.

Management/Decision MakingApplies existing guidelines and procedures to make varied decisions within a department. Uses sound judgment and experience to solve moderately complex problems based on precedent, example, reasonableness or a combination of these. If serving in single community, may directly supervise associates who are certified to pass medications. No supervisory duties if serving multiple facilities.

Knowledge and SkillsPossesses extensive knowledge of a distinct skill or function and a thorough understanding of the organization and work environment. Has working knowledge of a functional discipline. Basic typing skills along with a working knowledge of personal computing and word processing software are required, preferably in a Microsoft Windows environment. Ability to read and interpret documents. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or associates of company. Ability to effectively communicate in English and understand and follow written and oral instructions.

In this position you will: * Manage patient care, to achieve cost-effective utilization of resources through the proactive coordination of services in order to move patients efficiently through the continuum of care * Assume responsibility for an interdisciplinary process which assesses, plans, implements, monitors the effectiveness of interventions to meet patients' treatment and transition of care needs Required qualifications for this position include: * Nursing degree/diploma required upon hire. * All Registered Nurses without a minimum of a Bachelor's degree in Nursing must obtain a Bachelor's degree in Nursing or higher within 3 years of hire. * Any previously agreed upon effective date for obtaining a Bachelor's degree in nursing between individual and facility will remain in effect. * Any collective bargaining contract with specific time frames for obtaining a Bachelor's degree in nursing will supersede the above point and will remain in effect. * Exceptions will be considered on a case-by-case basis at each facility by the Director of Care Management in consultation with the Human Resources leader. * A RN with 20 years of RN experience will not be required to obtain a BSN * RNs employed prior to 4/1/17 are encouraged to obtain a Bachelor's degree or higher degree in Nursing, but obtaining the degre will not be a condition of employment. * Current California Registered Nurse license * Current Basic Life Support Card ( BLS ) for Healthcare Providers sponsored by the American Heart Association * Minimum of one year experience within the past three years in an acute hospital setting as Registered Nurse. * Minimum of one year experience as a Discharge Planner or Case Manager, succesful completion of a minimum of 24 CEUs in Care Management within the past 12 months, or successful completion of the Transition into Practice s( TIP ) program for Care Management * Strong clinical knowledge base * Advanced knowledge of McKesson's InterQual SI/IS Criteria Preferred qualifications for this position include: * Bachelor's Degree in Nursing ( BSN ) strongly preferred * Masters Degree * Minimum of 3 years experience in case management, discharge planning, and/or utilization review About the hospital you will serve: Providence Saint John's Health Center in Santa Monica, California, is part of Providence Health & Services, an integrated, not-for-profit network of hospitals, care centers, medical clinics, home health services, affiliated services and educational facilities in the western United States.
