Job Description:

As an RN Case Manager, you will be responsible for managing and delivering direct nursing care to PACE (Program of All-inclusive Care for the Elderly) participants in a variety of settings, including clinic, care center, home, virtually, SNF, and beyond. You will work collaboratively with the Interdisciplinary Team (IDT) to complete assessments and drive forward participant care plans.

Key Responsibilities:

- Contribute to insightful patient assessments, comprehensive care plans, delegation of responsibilities, and timely service determination requests as a critical member of the Interdisciplinary Team (IDT)

- Lead personalized care for a panel of participants and ensure progress on a participant’s plan of care by partnering with a medical provider and being the point person for the IDT

- Deliver care with other team members wherever the participant is located (home, care center, virtual, ALF, SNF) in the areas of basic primary care, medication administration, wound care, oxygen therapy, and any other clinical needs in accordance with scope of practice

- Conduct nursing assessments, inclusive of physical, psychosocial and behavioral status

- Identify emergency situations and independently initiate therapies as needed within the scope of practice

- Educate participants, caregivers, and staff on how to personalize and carry out care plans and respond to situations that arise in each participant’s life

- Monitor and evaluate therapeutic interventions, and recommend changes as needed to proactively improve health and wellbeing

Schedule and Shift Details:

This is a full-time position, with 40 hours per week, Monday-Friday, and occasional on-call coverage. Schedule may vary.
