• Job Duties:

• Asses clients' needs and develop individualized care plans.

• Coordinate and monitor services to ensure clients receive appropriate care.

• Setting:

• Office Setting separate, but connected to the hospital.

• Monday-Friday 8a-5p

Experience Level:

• 6 months of hospital facility experience
• 2 years floor experience
