Position Summary:

The primary focus of this position is on facilitating patient throughput. As a Case Coordinator at Hartford Hospital, assigned to a select inpatient population, you will assess patients for appropriate level of care; coordinate their care and services across the care continuum; identify risk factors; facilitate involvement of support services; perform utilization management functions; develop and implement transition planning activities; utilize and document aspects of care in our case management specific computer applications; and collaborate with other health care team members on process and quality improvement initiatives. You will gain the opportunity to become a key member of a collaborative nursing team, in which patients and their care will be your first priority.

  • The successful candidate will possess a CT RN license

  • BS in Nursing required

  • Three years acute-care, and diversified clinical nursing experience, including medical/surgical nursing is required; case management, home care, and/or managed care experience strongly considered

  • Excellent communication, negotiation and conflict resolution skills required.

  • Knowledge of computer applications and reimbursement systems preferred.

  • Certification in case management desired.

The Center utilizes a universal, evidence based, research informed, and policy driven approach to enhanced care coordination that not only meets the interrelated medical, developmental, behavioral, and social needs of children but enhances the care giving capacity of families. Under the general supervision of the Director of Care Coordination, The Manager of Care Coordination is responsible for the daily operations of care coordination throughout the continuum of care (inpatient, outpatient, community). Provides leadership direction and guidance to the team in the provision of care coordination services. Assists the Director of Care Coordination in achieving departmental and organizational goals and objectives. Employs universal, research-informed approach that increases family strengths, enhances child development, and decreases likelihood of child abuse & neglect (Strengthening Families™ Protective Factors Framework) Operations: Work with Director to review, revise, and monitor Strategic Plan and Playbook Engage in program development that expands the capacity of the Center and its work Contributes to development of performance standards and departmental initiates that support organizational goals. Participates in the development of guidelines and their incorporation into the care coordination process Ensures compliance with state and federal regulatory requirements in addition to Joint Commission standards Develops and maintains departmental policies, procedures, protocols, and annual educational activities Manage care coordination meetings, monthly team meetings, trainings, in-services, lead the development of the annual care coordination forum Interfaces with internal departments, medical staff, community partners, and third party reviewers to ensure regulatory and contractual requirements as well as achieve optimal operational efficiency Supervision: The Manager of Care Coordination will supervise all Care Coordinators and Administrative personnel. Manager will partner with clinical team leads to support and supervise nurses and social workers With support and supervision of the Director and in conjunction with the clinical team leaders, performs annual performance evaluations and provides ongoing coaching and mentoring of staff. Initiate and monitor team members on performance improvement plans Interviews and recruitment of new care coordination personnel Data Management: Monthly review of care coordination outcome data to identify and make recommendations related to specific populations or diagnoses, processes, metrics, ROI (return on investment), gaps in care, etc. Operations and Grant Management: Monthly review manage, monitor, evaluate operational budgets. Manage grant portfolio: manage and monitor grant deliverables, create grant reports, manage monthly meetings with the Office of Grants and Sponsored Programs Continuous Quality Improvement: Manage/monitor Daily Management System and Key Performance Indicators, conduct PDSA (Plan Do Study Act) cycles, conduct data and process audits Collaboration: Collaborates/partners with community/state agencies to increase effectiveness, efficiency & health outcomes while decreasing duplicity, redundancy and cost Support the Clinically Integrated Network by providing technical assistance, training, and support to increase the capacity of Care Coordination in the community Research and Innovation: Engages in innovation/research activities and develop annual care coordination forums.
