We're looking for someone to join our team as a RN, Transfer Center Coordinator who:

Coordinates patient care needs by facilitating patient placement in the appropriate nursing unit utilizing pre-established criteria and consulting with members of the interdisciplinary team. Receives and processes requests for assistance with acute patient transfers and/or admissions of patients from medical facilities.

Essential Functions:

  • Performs telephonic patient triage, discharge follow-up with physicians and patients and disease management programs. Coordinates with other clinics and hospitals, assisting with physician consultation, transfer and coordination of patient ground/air transport. Collaborates frequently with Emergency Room (ER), and Post-Anesthesia Care Unit (PACU) and bed boards to keep abreast of possible admissions/transfers.

  • Collaborates with the Bed Assignment/Staffing Group, Patient Placement Coordinator or Administrative Supervisor per shift to determine census and acuity. Collaborates to resolve bed availability challenges. Receives and obtains the admission orders for admissions and ensures that orders are sent to the appropriate nursing unit Documents coordinator and patient interactions in electronic or hardcopy chart in a manner consistent with hospital standards. Creates reports from patient data such as volumes and outcomes.

  • Performs other job-related duties as assigned.

You'll be successful with the following qualifications:

Education:

  • Bachelor's Degree (BSN): Preferred

Work Experience:

  • Experience as a lead or charge nurse or with placement or care of critically ill patients: Preferred

  • Experience working with electronic medical records (EMR): Preferred

Required Licenses/Certifications:

  • Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification: Preferred

  • Registered Nurse (RN) licensure in the state of practice: Required

Now is your chance to apply your passion to our mission.</p><p>We're looking for someone to join our team as a Sr. Pharmacy Technician who:</p><p>Responsible for medication preparation, dispensing, restocking, delivery, etc. in a specialty area applying specialized knowledge and skills. Fills the unit dose cassettes from the patients profile on a daily basis. <br><br> Delivers the cassettes to the nursing units. Serves as a primary training resource for new technicians and assists in evaluating their progress as directed. Assists in preparing schedules and setting work priorities.</p><p>Essential Functions:</p><ul><li><p>Processes prescriber medication orders under a pharmacist's supervision and in compliance with department policy. Fills medicine requests accurately including cassette fills and floor stock. <br><br> Delivers cassettes to respective nursing stations. Provides basic troubleshooting phone calls. Answers telephone calls and handles inquiry into some phases of unit dose procedure.</p></li><li><p>Maintains floor stock of nursing areas and other medications which includes checking for accuracy, timely restocking, removing expired medications and rotating stock. Prices and stocks incoming deliveries. <br><br> Notifies Pharmacists of out of stock medications and appropriately updates the out of stock board. Restocks returned medications from nursing units/medication areas.</p></li><li><p>Prepares IV medication products within the guidelines of applicable laws and regulations.</p></li><li><p>Checks in orders from wholesaler and suppliers. Distributes appropriately the stock and file paperwork; processes returns and back orders in a timely manner.</p></li><li><p>Performs other job-related duties as assigned.</p></li></ul><p>You'll be successful with the following qualifications:</p><p>Education:</p><ul><li><p>High School Education/GED or equivalent: Preferred</p></li><li><p>Associates / Technical Degree or equivalent combination of education/related experience: Preferred</p></li></ul><p>Work Experience:</p><ul><li>One year's pharmacy technical experience: Preferred</li></ul><p>Required Licenses/Certifications:</p><ul><li>Registration with the State Board of Pharmacy: Required (except HI)Pharmaceutical Technician Board certification: Preferred</li></ul><p>Adventist Health is committed to the safety and wellbeing of our associates and patients.
