Job Summary

The Care Coordinator will serve as the contact point, advocate, and informational resource for patients diagnosed with C. difficile infection. They will establish a relationship with eligible patients prior to discharge, follow-up with patients for up to 56 days to provide patient education, ensure access to medications, and act as a resource for patients should they have any signs and symptoms of recurrence. They increase comprehension through culturally and linguistically appropriate education and create and promote adherence to a care plan that is developed in coordination with the patient, primary care provider, and family/caregiver(s).

Job Requirements

Minimum Education Requirement :

Graduate of an accredited school of nursing.

Minimum Experience Requirement :

Care coordination and/or case management experience is desirable.

Evidence of essential leadership, communication, education, and counseling skills. Proficiency in communication technologies (email, cell phone, etc.) Highly organized with ability to keep accurate notes and records. Experience with health IT systems and reports is desirable

Local knowledge about and connections to community health care and social welfare resources is desirable

Must possess and demonstrate knowledge of the growth and development of varied age groups, neonatal to geriatric. Demonstrated ability to handle confidential hospital information.

Well developed critical thinking skills. Knowledge and understanding of medical terminology. Ability to communicate effectively with medical professionals of all disciplines.

Clear verbal and written communication skills, excellent phone etiquette. Ability to identify potential and actual patient occurrences that may be reportable incidents or those requiring further internal investigation.

Minimum License Requirement :

Current NYS licensure as an RN. BLS certified.

VIEW JOBS 5/29/2019 12:00:00 AM 2019-08-27T00:00 POSITION SUMMARY: The Care Coordinator RN works to eliminate barriers to care for (primarily) high risk patients and prevent avoidable readmissions. He/she works as part of an integrated team with Population Health, Care Coordination, Providers, Clinic Administrators, Health Information Management Program Manager, and Billing to ensure smooth continuum of care for identified patients. This includes managing a panel of high risk patients to ensure they are maximizing the resources available to them, making referrals and follow-up appointments, making telephone calls to patients, assisting patients in making and attending scheduled appointments, assessing patients to coordinate care with other healthcare providers, working with other healthcare providers to transition care as needed, and directing inquiries. The Care Coordinator RN may also conduct in-home or hospital visits to high risk patients to support successful transitions back into the home and to prevent readmissions or avoidable ER usage. QUALIFICATIONS: Current New York State RN license with proven knowledge of patient care, access, and compliance regulations. A minimum of 3-5 years of clinical or quality program management experience preferred. Supervisory experience preferred. Has an excellent interpersonal and verbal communication skills. Is skilled with computers and computer applications. Gives attention to detail and possesses problem resolution skills. Has a positive attitude and high energy, is a quick learner, is adaptable to change and is resourceful and proactive when identifying and resolving problems. NYS drivers license; reliable transportation; ability to lift equipment up to 30 pounds.
