We are seeking a Care Coordinator RN to perform care coordination within the boundaries of the company's policies and procedures to ensure appropriate care is delivered in a timely manner and within the appropriate setting for members. Interact with the member, family and care providers to develop, coordinate and monitor the member's treatment plan.

Primary Functions:

  1. Investigates opportunities for health care alternatives and prepares cost effective plans to offer patients, families, and providers.

  2. Identifies health services vendors to use in Care Coordination and coordinates with providers for member's benefits as well as identifies and utilizes community resources that may be available to member.

  3. Works closely with members, families, and providers, typically by telephone in order to outline a clear, proactive plan of care with patient goals and objectives and measurements to attain the same.

  4. Effectively use the company's internal systems to accurately determine eligibility, benefit plan, physician networks associated with the member's plan. Documents accurate, readable, narrative notes related to cases and identifies and researches any problems and offers reasonable solutions.

  5. Maintains an acceptable caseload within the guidelines outlined within the department's policies and procedures and works assigned schedule to maintain phone coverage between the hours of 7am - 5:30pm, this includes closing cases in appropriate timeframe per department standards and maintain RN audit scores at or above the 90% benchmark.

  6. Applies clinical expertise and judgment to ensure compliance with medical policy and criteria of for accepted standards of care while performing Utilization Review and Service Authorizations for members of assigned employer groups.

  7. Consults with physician advisors to ensure clinically appropriate determinations.

  8. Refers appropriate cases to Case Manager RNs for assessment.

  9. Provides approvals and denials of requests in a professional and positive manner.

  10. Reports on any case entered on the High Dollar Report in Rounds meetings.

  11. Develops professional relationships with internal departments, including effectively communicating with support staff and customer service staff.

  12. Contributes to the ongoing efforts of the department to foster a collaborative and team approach to case load.

  13. Performs other duties as assigned.

Vibra Specialty Hospital of Portland is seeking an experienced Full Time Wound Care Coordinator - Registered Nurse - to join our team! The Wound Care Coordinator is responsible for developing, implementing, and evaluating plan of care of facility patients with wound care needs. Provide specialized consultation and education on wound, ostomy and continence issues for patients, families, nursing and medical staff. The provision of care includes direct care along with the delegation and supervision of all wound, ostomy, and incontinence care. Specific components of the Registered Nurse role include demonstrating clinical competence; managing patient care; improving quality of care; establishing professional relationships and utilizing professional skills; fulfilling responsibilities of the role of the hospital; and developing clinical expertise per specialty. Qualifications: * Current, valid, and active license to practice as a Registered Nurse in the state of employment required. * One (1) year experience in wound care required. * Current BLS certification from a Vibra-approved vendor required. * Certification in Wound Care required within 6 months of hire. Additional Qualifications/Skills: * Wound, Ostomy, Continence (CWOCN) certification preferred. * Two (2) years experience working in wound care preferred. * Two (2) years experience working in an acute hospital preferred. * Ability to project a professional image. * Knowledge of regulatory standards and compliance requirements. * Strong organizational, prioritizing and analytical skills. * Ability to make independent decisions when circumstances warrant. * Working knowledge of computer and software applications used in job functions. * Freedom from illegal use of and effects of use of drugs and alcohol in the workplace.
