The RN Care Coordinator is responsible for assessing, planning, facilitating and advocating for options and services through a continuum of care from point of contact through discharge on assigned patients.The RN Care Coordinator performs this role in such a manner as to meet the individuals health needs while promoting quality and cost effective outcomes. The positions emphasis will be on care coordination, communication and collaboration with nursing, physicians, departments within the medical center, insurers and post acute service providers to pace the care toward optimal outcomes within the appropriate level of care. The RN Care Coordinator will advocate for the patient and family by identifying and valuing patient choice, spiritual needs, cultural, language and socioeconomic barreris to care transitions . In addition, the RN Care Coordinator will protect confidentiality while striving to achieve high levels of patient satisfaction. The RN Care Coordinator consistently conducts the utilization review process and the discharge planning process as required by hospital policies, standards of practice and Federal and State regulations.   • Minimum of 3 years nursing experience in an acute hospital setting; Prior Care Coordination experience in a clinical or insurance setting is required.

  • BSN degree or experience equivalent preferred

  • CM certification preferred; A Masters Degree in nursing with a concentration in Case Management can serve as a substitute for the experience requirement.

  • Excellent customer service and presentation skills are a must; Strong interpersonal and written communication skills are essential.

  • Demonstrated ability to apply analytical and problem solving skills; Demonstrated ability to manage multiple tasks or projects effectively

  • Ability to work independently as needed with a high degree of detail orientation

  • Ability to work efficiently in a fast-paced environment with changing priorities

  • Current Registered Nurse (RN) License to practice professional nursing within the State of California.

  • If operational conditions permit, training a candidatewithout the required experience may be considered.

Essential Functions and Work Responsibilities * Oversee and coordinate the development and completion of the participant plan of care in accordance with current Federal and State regulations, and guidelines that govern the participant assessment, to include the policy and procedures of InnovAge. * Present as the Chairperson of the Interdisciplinary Care Plan Team by coordinating the comprehensive participant assessment and care plans (i.e. represent the RN as well as facilitate the care plan meetings in their respective centers) * Tracking of completed care plans for compliance and maintaining the Care Plan schedule spreadsheet. * Edit care plans for legibility * Close care plans for printing * Organize and implement new or updated changes to plan of care involving the assessment and care plan process – specifically any policy and/or regulation changes relating to care planning * Facilitate Care Plan discussion for the Interdisciplinary Team by engaging the team in conversation and documentation needed to improve or maintain the participant's functional abilities at the highest practicable level (including auditing EMR, having team discussions in Morning Meeting, Care Planning, mini teams, etc. to ensure documentation, diagnosis, etc. match up and have corresponding coverage in the care plan). * Oversee the assessment and care plan process for timeliness * Develop plan / strategy to further engage and involve the family and participant in the care plan process * Participate via attending various committees of the center for quality Assessment and Assurance as required, as appointed by the Clinic Service Manager or Center Director * Work collaboratively with all new InnovAge teams to encourage timely, accurate and quality assessments and care plans – This position is responsible for timeliness of care plan audit documentation and completion – specifically scheduling, monitoring for timeliness, compliance, etc. * Develop a plan to provide Care Plan training for all new hires at their respective center as well yearly training for all other disciplines * Understand , comply with and promote all rules and regulations regarding participants rights, promote positive relationships with participants, families, visitors and regulators, * . * . * Comply with all Company policies related to safety and infection control procedures including the proper use of mechanical lifts, gait belts and personal protective equipment.
