RN Care Coordination Liaison Full Time Eves

POSITION SUMMARY:

Coordinates the care for patients throughout the organization from both a discharge planning and utilization review perspective on the evening shift. Performs concurrent utilization review activities on all patients handed off from the previous shift.

Collaborates and educates the medical staff on appropriate utilization review guidelines and documentation in accordance with CMS regulations and Commercial payers guidelines. Monitors and intervenes on LOS, resource utilization and denial management/ prevention. To coordinate and facilitate patient care through the Valley Hospital as it relates to a multidisciplinary approach to patient care and utilization of resources.

Expedites discharges throughout the organization based on hand-off and facility need. Provides support to the MD and RN team on the evening shift to assist in all facets of Care Coordination.

EXPERIENCE:

Minimum 5 years' experience in clinical nursing, minimum of 3 years in case management/ utilization.

EDUCATION:

Bachelor's degree (BSN) in nursing required. ANCC case management certification preferred. Expectations: achieve ANCC case management certification within two years (effective 2016), or when requirements for sitting for the exam case management exam, as stated by the ANCC

SPECIAL SKILLS:

Registered Nurse, Licensed in State of New Jersey, Current acute care clinical knowledge with broad-base medical / surgical background. Demonstrates effective interaction and communication skills.

Strong critical thinking, clinical reasoning skills and effective problem solving. Ability to perform work accurately and pay attention to details, often changing from one task to another without loss of efficiency or composure. Ability to function competently in stressful situations and changing work environment, Ability to work collaboratively within the health- system with patients and families; and with multidisciplinary team members (i.e. , physicians, Rehab, Respiratory, DI, Lab, etc.). Ability to utilize effective time management to set priorities, perform job related responsibilities, and respond quickly to emergencies. Ability to use critical thinking and clinical reasoning skills effectively problem-solve and provide case management services.

EDUCATION: Bachelor`s degree in Nursing required. Current/valid State of New Jersey license (or legal permission to work from the New Jersey Board of Nursing). Completion of general nursing orientation, CPR certification and unit based orientation. SPECIAL SKILLS: Physical and cognitive requirements: Position's physical requirements necessitate a full range of body motion including but not limited to, physically assisting patients, requiring stretching, bending, lifting, sitting, kneeling, stooping, standing, walking, pushing and pulling. Position is classified according to the USDOL as Very Heavy Work which is defined as exerting in excess of 100 pounds of force occasionally, and/or in excess of 50 pounds of force frequently, and/or in excess of 20 pounds of force constantly to move objects. The National Institute of Safety and Health (NIOSH0 guidelines, which recommends that the average worker not lift more than 51 pounds without assistance, will be followed. Manual and finger dexterity and hand-eye coordination are also required as is constant talking and listening in English. Must demonstrate effective oral and written communication skills, including speech clarity, understanding the information relayed, deductive and inductive reasoning skills (the ability to apply general rules to specific problems to produce answers that make sense and the ability to combine pieces of information to/from general rules or conclusion respectively). Considerable walking, standing, sitting, reaching, filing, typing and photocopying required. Ability to perform work accurately and pay attention to details, often changing from one task to another without loss of efficiency or composure. Ability to function competently in stressful situations and changing work environment related to changing needs of patients with acute, chronic and complex and/or terminal disease processes. Ability to work cooperatively within health system, multidisciplinary team members (i.e. MD's, Rehab, Respiratory, DI, Laboratory, etc). Ability to utilize effective time management skills to set priorities to perform responsibilities, problem solve and deliver care.
