Position Title: RN, Care Coordination Department: Social Services


Purpose:


Concurrently and retrospectively ensures the patient#s progress in the acute and skilled episode of care through post discharge and is quality driven while being efficient, cost effective, with a safe discharge plan.

Works with physicians to facilitate effective and efficient transitions through the process of hospitalization.

Works collaboratively with all members of the multi-disciplinary team to ensure patient needs are met and care delivery is coordinated across the continuum, as well as appropriately reimbursed by payers, and that resources are efficiently and effectively utilized. Serves as a liaison between the hospital and community agencies or facilities for the exchange of clinical and referral information.

Provide support for Emergency Department staff and patients.

Assist with set up of mobile meals. Coordinate with spiritual care as needed.

Provides support to staff, physicians, families, patients, and community members relating to child and dependent adult abuse, end of life care, and other community and facility needs.

Monitors performance improvement in accordance with Joint Commission standards and CAH standards. Provides services in role of Care Coordinator as related to telepsych services, as designated.

Supervisory Responsibility:


Guidance: The job requires the provision of occasional guidance and training to others. The job does not have formal or official supervisory responsibilities.


Materials Responsibility:


Very Limited.

Work requires very limited responsibility for material resources.

Examples of resources could include personal work materials, supplies or equipment, or very small amounts of cash. The employee has a very limited or indirect amount of control over these resources.

Although human error might require the repair or replacement of materials, usually the cost of correcting these errors is minor.

The variety and volume of resources is also very limited. Problems associated with material resources are very uncomplicated.

More than 2 years experience required.

Communications are often extremely difficult or stressful in nature.

Contact with others involves highly complex and sensitive topics. The job requires extremely well developed interpersonal skills for dealing with a range of complicated problem situations. The job requires the use of diverse communication techniques.


Working Conditions:


There is limited exposure to adverse environmental conditions.

Some undesirable or unpleasant environmental characteristics may occur but the physical environment is generally safe and there is minimal health risk.

No safety equipment or unusual precautions are required. The occasional exposure to these minor adverse conditions would be limited to 35% or less of the work day.
