Providence is calling a Registered Nurse (RN) Assisted Living (Nursing Administration, Days, 0.0 FTE) to Providence Horizon House in Anchorage, Alaska. In this position you will:

  • Employs Nursing Professional Practice: Assesses, diagnoses, plans, implements and evaluates patients' care. Develops, updates and coordinates the patient's plan of care to achieve patient goals and to optimize outcomes and transitions across the care continuum. Monitors, records and communicates patient condition as appropriate. Performs and maintains currency of essential competencies as required by specific area of hire and populations served

  • Advocates: Effectively advocates for ethical and holistic care by partnering in care planning to promote the autonomy, dignity, rights, values and beliefs of those we serve

  • Facilitates Learning: Fosters a learning environment for patient/families, nursing, and other members of the healthcare team including students; facilitates formal and informal learning for patient/families, nursing, other members of the healthcare team, and community.

  • Aligns Practice with Safety and Quality: Accountable for safety, identifies and corrects problems, and integrates evidence, and best practice into nursing care. uses data and evidence to improve patient outcomes. Aligns patient safety, self-safety and environmental safety as equally important for best outcomes

  • Engages in Professional Development: Engages in ongoing professional development; practices to the top of license within the legal parameters of the Nurse Practice Act, the Nursing Social Policy Statement, the ANA Code of Ethics for Nurses, and specialty standards. Promotes a culture of inquiry that explores, integrates and disseminates research and evidence based practice

  • Leads in the Practice Setting & Community: Leads and coordinates teams, appropriately delegates, coordinates care and collaborates with others as equal members of the inter-professional care team to integrate nursing knowledge. Models professional behaviors as a representative of the nursing profession

  • Delegation: Give on-the-job direction including appropriate delegation, input and coaching to assigned members of patient care team

  • Complete initial and annual Competency Plan for assigned job and department

Required qualifications for this position include:

  • Nursing degree/diploma from an accredited nursing program

  • Unencumbered RN license in Alaska

  • BLS from AHA required upon hire

  • 2 years experience in the healthcare field

Preferred qualifications for this position include:

  • Bachelor's Degree from an accredited nursing program
  • 1 year experience as an RN

Horizon House is an assisted living community where most residents have more autonomy than in a long term care facility. There are 60 apartments and two cottages; some with double rooms. The homelike atmosphere and two outdoor garden areas make for a pleasant atmosphere for residents and staff alike.

In this position you will: * RESIDENT CARE: Demonstrates understanding of and carries out individual resident care according to the philosophy and mission of Providence Health & Services, Providence Horizon House, Eden philosophy, and the Resident Service Agreement. Assists residents with personal care functions including: dental and mouth care, bathing, dressing and grooming, hair care, nail care, shaving, bowel and bladder care, and other personal and hygiene care. Assure all resident calls for assistance are answered promptly. Promotes autonomy of residents in decision-making * REPORTING/DOCUMENTATION: Measure and records vital signs, weights, bowel records, and other required documentation clearly. Reports changes observed in residents to the Nurse in a timely manner. Performs safety checks as indicated in service agreement * PERSONNEL: Works in a spirit of cooperation with new and experienced Resident Guide I and Resident Guide II with unfamiliar routines. Communicates with coworkers at all levels to meet needs of residents * Assists with and/or performs restorative and rehabilitative procedures as outlined and assigned by the service agreement and Physical Therapy providers * Assists with transporting of residents and participating with activities as needed * Assists with moving, positioning and transporting residents into and out of beds, chairs, wheelchairs, lifts, etc. * Assist with transporting residents to Dining Room and/or serving meals, and other nutritional snacks per Recommended Daily Allowance (RDA) guidelines and stated preferences of the resident. Responsible for following all food safety rules, and monitoring refrigerator and freezer temperatures * Keep all areas of Horizon House clean, performing routine housekeeping duties. Make occupied and unoccupied beds and change bed linens. Launder clothing, linen and other articles * Maintains confidentiality of all resident medical and personal information obtained while performing the duties of the job * Follow established safety precautions and observe, monitor and intervene in or report to the Nurse any unsafe conditions. Report accidents and incidents when they occur. Report any resident abuse or suspected abuse to the Nurse immediately Required qualifications for this position includes: * Certification in first aid and CPR within four (4) months of hire * Must hold a current State of Alaska CNA certification * New CNA graduates and candidates that hold a current, unencumbered nurse assistant certification or registration from another U.S. state must obtain the State of Alaska certification within four (4) months of employment Preferred qualification for this position include: * Six (6) months experience as a nurse aide, nursing assistant or private care giver About Horizon House Horizon House is an assisted living community where most residents have more autonomy than in a long term care facility.
