Position Overview:

  • This position is the primary point of contact for the Medical Director and physicians regarding clinical coordination with SV nursing services. This position assists in long-term care operations, analyzes performance improvement results, develops action plans, and communicates information related to long-term care clinical regulatory compliance, and performance improvement activities to applicable long-term care (LTC) personnel or committees as directed by the LNHA. This position initiates and creates databases, which assist LTC clinical and administrative decision-making and formulation of applicable policies. This position maintains the LTC compliance, administrative and clinical manuals, creates committee schedules, agendas as well as other duties as assigned by the LNHA. Effectively develop, implement, and maintain overall governance of nursing services, staff development, QAPI, and Infection Prevention programs in accordance with CMS and NYS DOH guidelines.

What you'll do:

  • Effectively develop, implement, and maintain overall governance of nursing services, staff development, QAPI, and Infection Prevention programs in accordance with CMS and NYS DOH guidelines.

  • Assumes role of acting administrator in absence of nursing home administrator

  • Utilizes performance measurement systems to help identify key quality/cost opportunities; develop performance measures for effectiveness of performance improvement activities

  • Plan development. Provide education, leadership, guidance and assistance to clinical team and individuals in the development of meaningful performance improvement plans

  • Facilitate sustainable performance of project teams and quality initiatives, including Root Cause Analysis action plans; work with departments, sites and staff representatives to communicate improvements broadly across the organization; cultivate broad participation in performance improvement activities.

  • Assist with daily operations of the clinical team and support the Director of Nursing, Assistant Director of Nursing, Medical Director, and providers

  • Perform basic analysis of data and will create, prepare, and deliver reports to various departments as directed by nursing home administrator

  • Develops and implements facility Safety Committee and Safety program in relation to New York State and Federal nursing home regulations

  • Implements and manages clinical regulatory compliance and directives for nursing home staff development as outlined by NYS Dept. of Health and CMS/Federal regulations

  • Develop, implement and maintain facility policies and procedures. Serves as the chair of daily clinical rounding and daily huddles

Minimum Qualifications:

  • RN License in the State of New York
  • 3-5 year of Long Term Care experience, or 2 plus years' experience as Director of Nursing or Assistant Director of Nursing, or 2 years' experience with long term care Quality Assurance and Performance Improvement programs/methodologies.

Physical Requirements:

  • Repetitive use of hands and fingers (e.g., Use of computer keyboard); may require lifting and carrying light loads (up to 40 lbs), including boxes, equipment, IV solutions and stooping and kneeling (e.g., to pick up items from the floor, to remove and replace items on lower shelves and to file documents in lower file drawers); sitting, walking, or standing for long periods of time may be necessary; must be able physically to operate the equipment used for the job.
