Under general supervision of the Director of Nursing or Nurse Supervisor, the position directs the provision of resident care services for a particular shift and/or unit and supervises the nursing staff assigned to that unit/shift, with responsibility and accountability for assuring that quality nursing care and service to facility residents/families, compliance with applicable laws and regulations, and adherence to company policy/procedure occur on that unit/shift. Performs direct nursing services to residents, in accordance with individual plans of care and physicians' orders. Technical assistance, professional guidance, and quality monitoring are provided by administrator, director of nursing, regional consultants, and other nursing professionals on staff, Medical Director.


  • Current registration and licensure in the state of Iowa as an RN. Must be in good standing with the Board of Nursing.

  • Current, valid CPR Certification.

  • Knowledge of federal and state long-term care regulations and laws.

  • Management and supervisory skills.

  • Functional Literacy in written and spoken English

  • Proficient interpersonal relationships and communication skills

  • Ability to follow written and oral instructions

  • Complete assignments timely, completely and accurately

Essential Functions

  • Follow established standards of nursing practices and implement facility policies and procedures.

  • Assume responsibility for unit/shift staff compliance with rules, regulations, standards of practice and facility policy and procedure; assure that residents receive needed nursing care and services on that shift, according to plans of care and physicians' instructions.

  • Interpret existing policies and procedures, rules and regulations to nursing assistants, restorative nursing assistants, residents, families, physicians and agency personnel.

  • Supervise and evaluate all direct resident care and initiate corrective action as necessary.

  • Document resident care provided and resident's response or lack of response to care provided.

  • Obtain report from nurse being relieved and record sufficient information to implement appropriate follow-up action as necessary.

  • Provide report to nurse coming on duty, including sufficient information for follow-up action as necessary.

  • Take appropriate action to assure adequate staffing to meet residents' needs.

  • Conduct resident rounds daily, reporting problems to nursing supervisor and initiating corrective action.

  • Identify resident problems and emergency situation and initiate emergency care and "life-saving" measures in the absence of a physician.

  • Provide supervision to Nursing Assistant providing direct resident care.

  • Follow staffing guidelines and make nursing assignments according to qualifications of staff and characteristics of the resident population.

  • Plan daily work assignments and schedule based on residents' needs and available resources, effectively communicate to unit/shift staff, DON, ADON, and staffing clerk, and monitors staff compliance with assignments and schedule.

  • Check meal trays and nourishment to ensure proper diet is served and residents are assisted as necessary.

  • Supervise the application and removal of all restraints and devices used to position and enable residents to function as independently as possible.

  • Order, receive and store medications appropriately.

  • Monitor/control administration of medications according to applicable regulations.

Authority is delegated to the individual in this position to:

  • Take appropriate action to assure adequate staffing to provide resident care.

  • Supervise and assess resident care and take appropriate action.

  • Implement facility procedures to provide resident care.

Position Supervises: CNA's, CMA's, Non-CNA's, LPN's and other nursing staff as applicable

Perform prescribed activities directed under the patient's physician's care which may include; patient assessments, feedings, respiratory system management, medication administration, activities of daily living, and therapy programs. Assist in care coordination with care team. May be responsible for transportation of patient. Responsibilities Key Accountability * Delivers quality patient care consistent with Home Health Conditions of Participation, qualifying home care criteria, payer coverage criteria, and providing visits/treatments consistent with the plan of care. * Communicates with Registered Nurse Case Manager/Clinical Supervisor and participates in care coordination to achieve high quality care. * Performs other duties as requested by the Clinical Manager/Clinical Supervisor to facilitate smooth and effective operations of the organization. * Maintain accurate and timely documentation of clinical records and time entry. * Monitors the patient care process to evaluate and improve care, collaborating with providers and other health care workers to provide comprehensive health assessment, treatment and follow up evaluation. * Provides patient, family, and/or caregiver education and information pertinent to diagnosis and safety issues. Key Accountability * Train and orient new staff as assigned. Key Accountability * Employee is responsible for completion of communication tasks and activities in a timely manner. This would include, but is not limited to: responding to email, voicemail or telephone messages, promptly, accurately, and professionally; attending staff meetings and Pediatric Learning Lab/skills fair as scheduled; asking questions of team members and supervisors when needing clarification about various day-to-day issues or patient needs; and reviewing employee communication pieces, to stay aware of UnityPoint at Home programs and initiatives. Basic UPH Performance Criteria * Demonstrates the UnityPoint Health Values and Standards of Behaviors as well as adheres to policies and procedures and safety guidelines. * Demonstrates ability to meet business needs of department with regular, reliable attendance. * Employee maintains current licenses and/or certifications required for the position. * Practices and reflects knowledge of HIPAA, TJC, DNV, OSHA and other federal/state regulatory agencies guiding healthcare. * Completes all annual education and competency requirements within the calendar year. * Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff. Takes appropriate action on concerns reported by department staff related to compliance. Qualifications Minimum Requirements Preferred or Specialized Education: * Graduate of State Board approved program for Registered Nurses or Licensed Practical Nurses and current license to practice nursing according to the state(s) where service is provided * Masters education in health related * BSN or BS in health related field. Experience: * Strong computer skills. * Ability to understand and apply guidelines, policies and procedures. * Preferred experience working in clinical healthcare position. * Previous pediatric home care experience. License(s)/Certification(s): * Maintenance of an acceptable driving record (i.e., no "OWI" convictions in last 7 years or no more than 3 moving violations in last 3 years). An unacceptable driving record may result in suspension, termination or transfer to another position not requiring the ability to drive.
