Job Summary

The Access Care Manager is responsible for managing the entry and coordination of patients into the hospital. The Access Care Manager determines patient status and initiates first level clinical reviews according to hospital approved clinical criteria and in accordance with the Care Management Program's Utilization plan and payer specific requirements. He/she will address issues related to the planning, execution and management of bed placement of all admissions from all entry points into the hospital (e.g. EC, direct admits, transfers, etc.) in accordance with the clinical standards and guidelines set forth by Phoebe Putney Memorial Hospital. The Access Care Manager collaborates with Physicians and staff in the EC to identify discharge planning needs that can be safely and effectively met in alternative settings. The Access Care Manager performs admission clinical reviews for appropriate level of care and patient status, and is responsible for ensuring that patients are admitted to the most appropriate bed available to meet their clinical needs using established criteria for admission. All patient transfer activities will follow EMTALA regulations and guidelines. All activities will be performed in support of the strategy, vision, and values of Phoebe. General requirements

  • Performs all job responsibilities in alignment with the core values, mission and vision of the organization

  • Maintains current knowledge in present areas of responsibility (i.e., self education, attends ongoing educational programs)

  • Attends staff meetings and completes mandatory in-services and requirements and competency evaluations on time.

  • Demonstrates competency at all levels in providing care to all patients based on age, sex, weight, and demonstrated needs. For non-clinical areas, has attended training and demonstrates usage of age- specific customer service skills.

  • Wears protective clothing and equipment as appropriate.

  • Adheres to the hospital and departmental attendance and punctuality guidelines

Working Conditions

  • General environment: Works in a well-lighted, air-conditioned area, with moderate noise levels.

  • May be exposed to high noise levels and bright lights.

  • May be exposed to limited hazardous substances or body fluids, or infectious organisms.

  • May be required to change from one task to another of different nature without loss of efficiency or composure.

  • Periods of high stress and fluctuating workloads may occur.

  • May be exposed to physical altercations and verbal abuse

  • May be scheduled as needed including overtime

Education Requirements

  • 2 year / Associate Degree in Nursing (Required)
  • 4 year / Bachelor's Degree in Nursing (Preferred)

Experience Requirements

  • 4 - 5 years progressive clinical and basic management experience(Required)

Certifications and Licensures

  • Required Certifications/Licensures : Registered Nurse (RN) with current Georgia license.
