Position Highlights:

  • Hours: Hybrid
  • Monday-Friday 8:30am

  • 5:00pm.

What you will do:

1.Researches resource options for patients/families referred by social service staff. Clarifies request and translates the identified needs into appropriate options, informed by understanding of the pertinent social support system(s).

2.Educates patients and colleagues about applicable resources: their benefits, eligibility criteria, how to apply and advocacy tips.

3.Completes or assists in completing certain applications with/on behalf of patients (e.g., MassHealth Transportation

  • PT-1, The RIDE 30 Day medical necessity, HIV Drug Assistance Program)

4.Updates our patient handouts and staff reference materials.

5.Orients new social service staff about our services.

6.Notarizes patient signatures on urgent documents as appropriate (e.g., Power of Attorney).

7.Attends internal and community meetings to stay current with changes and to share our knowledge.

8.Other duties as assigned, including website management, written educational materials, special assignments and projects.

What you will need:

1.Bachelor's degree in Social Work or related field preferred.

2.Demonstrated interest in social work and/or health care.

3.Excellent customer service, problem-solving, collaboration, and teaching skills.

4.Excellent organizational skills, ability to work independently and juggle multiple projects/tasks.

5.Excellent written and verbal communication skills.

6.Computer savvy. Proficient with Microsoft Word and Outlook. Microsoft PowerPoint and Excel helpful. Experience/interest in building web-page; HTML and/or Dreamweaver experience helpful.
