The Family and Community Engagement (FACE) Resource and Referral Specialist will promote meaningful family involvement within the educational system by providing training and support, building collaborative community relationships, and providing information about available service options and delivery systems for children with disabilities to a variety of stakeholders throughout the Mid-State Region. Collaboration and communication with partners from the community, families of students with disabilities, and educational organizations is essential.

The Specialist will interpret, plan and execute their work under the direction of the FACE Center Director. The Specialist will use well-developed analytical skills. The Specialist may provide input into strategic decisions that affect work plan and budget planning. The Specialist will be expected to independently perform assignments and apply their own experience and knowledge of special education practices and procedures and working with families of children with disabilities to achieve center objectives.

New Consult and Specialist Referral Coordinator POSITION STATUS: Non-Exempt, Full-Time SUPERVISION RECEIVED: Reports directly to Manager of HIRS dept TYPICAL PHYSICAL DEMANDS: Work may require sitting for long periods of time; also stooping, bending and stretching for files and supplies; occasionally lifting files or paper weighing up to 30 pounds. Requires manual dexterity sufficient to operate a keyboard, type at 30wpm, operate a fax machine/scanner, telephone and such office equipment as necessary. Vision must be correctable at 20/20 and hearing must be in the normal range for telephone contacts. It is necessary to view and type on computer screens for long periods and to work in an environment which can be stressful. TYPICAL WORKING CONDITIONS: Work is performed in an office environment. Involves frequent contact with staff, outside offices and patients. Work requires attention to detail and the ability to multi-task. It may require changing duties for the day with short notice. EXAMPLE OF DUTIES: (This list may not include all of the duties assigned) * Manage new referrals for all sites, and radiation. To include new consult requests from fax, phone, email and in-person requests. * Ability to schedule outside specialist as well as new consult appts within parameters ordered by providers. Schedules/reschedules these appointments as necessary and relays message to required staff. * Register New Patients in our G-4 system. * Notifies patient and referring office as to date and time of appointment by creating E-scribe NC letter, faxing to referring along with missing document request and mail new consult packet to include new consult paperwork, practice summary and letter with appointment information. * Retrieves records from the hospital portals for new patients being discharged for follow up. * Manages phone calls and returns calls as necessary to referring offices, outside specialists as well as patients. * Maintains exceptional professional demeanor and etiquette with all telephone and face-to-face contacts. * Creates and maintains contact list of referral specialists including each offices address, phone number and fax number. * Ability to E-file * Maintains current knowledge and Logins for G4 and necessary hospital portals * Provides support services to patients and medical staff. * Maintains patient confidentiality. * Performs related work as required. * Enhances skills through continuing education. * Attends required meetings and participates in teams as requested. PERFORMANCE REQUIREMENTS: Knowledge, Skills, & Abilities: * Knowledge of medical office procedures * Must possess excellent written and verbal communication * Must have the ability to read, understand, and follow oral and written instructions. * Must be a motivated, quick learner with the ability to work independently. * Must have the ability to multi-task under pressure and prioritize. * Knowledge of medical terminology is required. * Ability to establish and maintain effective working relationships with patients, employees, referring offices and the public. Education: High school graduation or GED Experience: Two years experience in the medical field. Has knowledge of medical terminology and excellent computer skills.
