Full-time Resource Coordinator/DON---Rehab needed at a top hospital in Savannah, GA!

Under the general supervision of the Manager of the unit, the Resource Coordinator/Skilled DON is responsible for facilitating teamwork, coordinating in conjunction with health care teams, organizing staffing, monitoring supplies/equipment, completing patient flow/scheduling, and functions as a change agent for the unit. Develops, motivates, assesses and negotiates as a change agent in an effort to enhance organizational effectiveness.

Assists the nursing home administrator to ensure skilled nursing unit regulatory compliance is followed, as well as participate in the skilled nursing unit meetings. Utilizes the nursing process to care for adult and geriatric patients based on current standards of practice. Functions within the permissible scope of nursing practice as defined by the Georgia Board of Nursing and the hospital system policies and procedures.

Additional Education Information

  • Graduate of an accredited school of nursing, BSN preferred. Additional unit specific education and/or certification may be required.
  • 2-3 years of nursing experience.

This is a grant fundedposition. Responsibilities * Providesystematic and accountable, quality improvement technical assistance to ChildCare Centers and Family Child Care Homes participating in the state quality rating and improvement system, Quality Rated, withinthe service deliveryarea. * Assist in administering mini-grants to eligible child care programsparticipating in the quality improvement/technical assistance process. Ensure adherence toBFTS mini-grant guidelines. * Conductassessments of the family child care provider and child care learning center todetermine success factors and assess readiness to receive quality improvementtechnical assistance services from the CCR&R; provide orientations andtraining for child care providers describingtraining and technical assistance process, expectations and resources available. * Develop a Quality Improvement Plan (QIP) with the provideror center directorbased on needs assessment information. * Assistin implementation of the QIP, which includes on-site technical assistance visits,telephone and office consultation, advice on selection of equipment, on-sitetraining, linkages to other community resources and professional developmentopportunities via the CCR&R, universities and technical colleges. * Providelinkages and resources to assist providers with enrollment and scholarshipinformation for institutes, conferences, and degree programs; provide linkagesto CCR&R community resources. * Participate in and encourage participation in localand national providerassociations and be familiar with national ECE accreditations. * Trackall technical assistance activities, record and maintain information and dataas assigned. Generate reports, provide feedback on outcomes, identify barriers,solutions, promising practices and make recommendations; prepare and presentprogress updates to CCR&R Executive Director, Assistant Director ofOperations, TA Manager and staff as needed. * Acquireand maintain proficiency in the Infant/Toddler Environment Rating Scale(ITERS), the Early Childhood Environment Rating Scale (ECERS), Family ChildCare Environment Rating Scale (FCCRS), School Age Childhood Environment RatingScale (SACERS), and Quality Ratedas assigned. * Conform with and abide by all regulations, polices,work procedures and instructions for supervising agency. * Develop and conduct providereducation and training; ensure the use of pre and post assessments & customer satisfaction surveys. * Maintain all records and fiscal reporting requirements as designated by the supervising entities. * Performsother duties as assigned to meet program needs. MinimumQualifications * BachelorsDegree Early Childhood Education, Elementary Education, Special Education,Family and Consumer Science or related field. * Minimumtwo years of experience in working in child care field (classroom teacher,administrator, family child care home provider, trainer, technical assistanceconsultant, etc.). * Strong understanding of early childhooddevelopment and childcare rules and regulations for the stateof Georgia. * Abilityto forge a mutually respectful partnership with persons served and theirfamilies in order to assist them in gaining the skills and confidence toaddress issues and problems they face. * Excellent customerservice skills, organizational skills, and oral/written communication skills. * Must be a self-starter, work well with minimal supervision, as well as the ability to work in partnership with other team members. * Must have intermediate knowledgeof Microsoft Officeincluding Word, Excel and PowerPoint; ability to use a networkeddatabase. * Mustbe able to travel to locations throughout assigned territory, valid Driver'slicense and appropriate insurance required. Preferred Qualifications (In addition to Minimum Qualifications) * Threeyears' prior experience in a child care setting. * Efficient,able to maintain schedule that is conducive to provider's needs. * Abilityto work in a team environment with a diverse group of people. * Exceptional abilityto react in a timely manner to requestsfor assistance, and requests from CCR&R AgencyDirector/Assistant Director. * Excellentorganizational skills, pays attention to details and deadlines. Physical Demands Theemployee occasionally lifts or moves objects of a light to medium weight. Workis typically performed in an office environment with intermittent sitting,standing, or walking in various settings. The employee must be able to lift upto 25 pounds.
