Overview and Qualifications

JOB SUMMARY: Coordinates the plan of care to skilled nursing and assisted living facilities and other post-acute providers and community resources in a cost-effective and time efficient manner.

Serves as a true patient advocate and a resource for Care Management Team, multi-disciplinary staff and physicians. Collaboratively engages with Care Management staff to ensure appropriate utilization of resources and smooth transition for facility placements in the community and region. Collaborates to achieve optimal patient satisfaction and positive clinical, financial and quality outcomes for assigned patient population across the system.

REQUIRED EDUCATION: BA/BS is related life/health science field.



PREFERRED LICENSE: Level I: NC Certified Social Worker, Accredited Case Manager (ACM)

REQUIRED EXPERIENCE: 3 years' experience in health care setting or local social service agency. BSW internship at Mission or other acute care hospital accepted in lieu of required experience.

PREFERRED EXPERIENCE: 5 years hospital/healthcare discharge planning experience.

Job Summary: This position is responsible for overall performance of their Operation Centers and supported contractor sites for work plan adherence, financial goals, resource utilization, customer commitments, and metrics by working closely with C&M Leadership, having ultimate accountability of the assigned area of responsibility. This leader is also responsible for setting direction and vision to the work optimization teams impacting Distribution work and overall effectiveness of the work flow, resource utilization and customer satisfaction. This work includes routine/service, 1st responder/trouble, customer and major projects. This position is responsible for leading and driving performance of their respective operations centers through various stakeholder workgroups and in close partnership to C&M leadership. This accountability is across all Distribution impacting areas. This Supervisor directly leads 8-16 work readiness specialists who are responsible for these aspects at the individual site locations. The direct reports are exempt level positions operating with this specific function and span across a wide geography. This position works with all support departments that impact Distribution work and makes sure impediments are addressed in a timely manner. They also have the ability to see trends and issues at a more global perspective (i.e. zone level) and should collaborate with the area GM for these trends, influenced by the various stakeholder groups. Responsibilities and Duties: Accountable for operational and financial performance of the Operations Center(s) Assigned: * Responsible for collaborating and driving the success of C&M leadership, to include the main objective of optimizing Ops Center performance and achieving goals assigned KPIs. * Responsible for work flow and compliance to process/procedures that foster the achievement of financial goals and improving unit cost of doing business at the Operations Center(s) assigned. * Review and action any negative trends/concerns with Financial performance of the Operation Center * Construction resource management for contractors and for Duke Energy employees working with Duke C&M leadership on authorized vacations, callout impacts, emergencies, vacations, etc. * Accountable for Financial and Operational Performance Dashboard (stand up with C&M Supervisor (people) and Specialist (work) * Awareness of all jobs scheduled, rescheduled, and enables their team to be empowered to make resource decisions to shift resources to get the work completed in consideration of financial impact * Oversight of daily Duke and Contractor work plan * Responsible for all customer commitments and operations center customer satisfaction for the operation center(s) assigned * Ensures Planner/Schedulers are engaged in decisions for any jobs to be rescheduled that impact customer commitments, instances where contractors are pulled from planned work and/or deviations from the work plan. * Reprioritization of work depending on daily dynamics. * Ensures safe and efficient utilization of all resources to meet the operation center requirements Root Cause evaluation and impediment removal of issues impacting Operations Center Readiness: * Review of trends impacting efficient work flow and operational effectiveness (awareness of jobs scheduled, potential resource issues, customer impact concerns, material and supply chain issues, etc.) * Validation of job site readiness around execution of the work, wasted trips for crews, root cause evaluation and removal of impediments * Ensures their team holds daily operation center stand ups/collaboration sessions to determine the success for the day and capture any impediments to efficiency. Understanding of why jobs did not get worked as planned and customer dates missed. * Controls and mitigates disruption to work plan and facilitates sharing of resources and work readiness specialists across assigned op center boundaries. * Feedback loop to stakeholders for drivers that impact ability to get work done. * Facilitates the removal of all impediments to operation center effectiveness, in close partnership with C&M leadership and direction. Build relationships with and provides feedback to operational center leadership, designers, schedulers, engineers, C&M supervisors, material planners, administrative/work management support, and fleet services. Documents feedback as appropriate or needed: * Promotes coordination with other entities to meet internal requirements and external requests. * Partners closely with Planners and Schedulers on the work plan and work flow execution. * Address trends and removes impediments influenced by various internal stakeholder groups. * Ensures the investigation for all escalated customer issues by working with the accountable department to ensure resolution. * Supports the investigation and resolution of claims, public damage and other issues with external customers and surrounding communities. Actively Participates as Member of Site/Zone Leadership: * Serves as backup as needed when supervisor is unavailable due to vacation, special assignment. * Serves in leadership position in afterhours storm response structure. * Actively participates on zone/system level safety team to develop and implement site safety improvement plans. * Participate in leadership capacity for off system storm restoration. * Train, mentor and develop employees to ensure on-going development and growth of employee member skills. Accountable for providing leadership development, direction, motivation and understanding of individual importance toward expected results: * Work with direct reports (exempt level positions) on career development and action planning to build upon current and future strengths * Ensure individuals/DRs displays safe and quality work habits and behavior and models the culture of safety and customer commitment * Access individual strengths and weaknesses to identify training needs * Track employee performance, applying corrective action and recognition procedures consistently * Ensures Specialists make sound, logical and timely decisions * Fosters and communicates confidence in the Work Flow Process, which includes receiving and communicating feedback for process improvements * Removes barriers that prevent work optimization team from being successful in all aspects of the work flow at assigned operations centers Basic Qualifications: * Minimum High School Diploma * 8 - 10 years of related electric utility distribution C&M Operations experience * Valid Driver's License Desired Qualifications: * 2 year or 4 year degree * Project Management experience * CDL License * Demonstrated effective written & verbal communication skills * Demonstrated effective problem solving and analytical skills * Demonstrated effective conflict management and negotiating skills. * Possesses experience with various Distribution C&M computer applications * Demonstrated motivational, presentation, public speaking skills. * Demonstrated ability to apply Environmental and Health & Safety Considerations into Project Execution Plan (ie.
