Job Summary This position is at Project HOME's newest program, the Maguire Residence, located in the Kensington neighborhood of Philadelphia. The chief responsibility of the Residential Services Coordinator is to provide collaborative recovery planning and follow through with residents and supports utilizing a recovery model. These services include keeping accurate records as well as working as an advocate for the residents in connection with a team.

Essential Duties and Responsibilities

  • Core Responsibilities

  • Build relationships and foster a sense of community. Encourage residents to participate and support various functions and opportunities at Project HOME

  • Design and implement recovery plans in the areas of recovery, health care, medication monitoring, money management, education, and employment which encompass short and long term goals, action plans and means for evaluation and revision.

  • Assist residents with learning to maintain lease obligations, independently or with the help of supports.

  • Assist residents with coordination of benefits and resources

  • Refer residents to BenePhilly and assist applicant with any related follow up needed (for example with DPW, SSA, unemployment).

  • Refer residents for employment and education services including but not limited to PHOME's Employment Services, the Honickman Learning Center (Adult Education) and partner with departments associated with resident employment and education goals.

  • Support residents through the completion of housing recertification process.

  • Meet as scheduled with the following individuals:

  • Participate in staff, team, and resident and special committee meetings.

  • Meet as scheduled in supervision with the Program Manager.

  • Participate in trainings and continuing education opportunities.

  • Facilitate Wellness Self-Management group and individual sessions with residents.

  • Plan and actively participate in site based and agency activities.

  • Intake, Orientation and Discharge

  • Utilize Critical Time Intervention best practices.

  • Complete all admission and discharge paperwork accurately.

  • Enter necessary information into electronic charting system, complete all assessments, and facilitate support groups and House Meetings.

  • Use the orientation process to inform new residents of the residential site, agency, services and neighborhood resources.

  • Assist residents with coordination of supports including referrals, linkage meetings and advocacy in the following areas: Addiction, Housing, Employment, Education, Medical, Mental Health and Natural Supports. Maintain records and reports, as required in a timely and thorough manner.

  • Ensure safe medication procedures including but not limited to: monitoring medications/medication scheduling, storage, and disposal.

  • Collaborate with residents and other supports to create appropriate discharge plans. Connect discharged residents to Alumni Services.

  • Alternate on-call responsibilities with other residential staff for emergencies. Work with residents and supports through crises, assessing the situation and preparing appropriate plans and interventions. Report and respond to incidents in a timely and appropriate manner.

  • Work closely with Property Management and other departments within Project HOME as needed and all other tasks as assigned.

  • Communicate effectively in a respectful, culturally sensitive and timely fashion. Develop and maintain cooperative working relationships with others. Encourage an open expression of ideas and opinions.

Minimum Qualifications

  • High School Diploma or GED and 4 years' experience with persons served; or an Associate's Degree and 2 years' experience; or Bachelor's Degree

  • Experience working with mentally ill/dually diagnosed clients and/or homeless population

  • Excellent verbal and written communication, organizational and interpersonal skills

  • Experience working with teams

  • Exhibits strong computer/technology literacy

Preferred Qualifications

  • Bachelor of Arts/Bachelor of Science Degree

  • Valid Driver's License

  • Work effectively with other outside agencies

  • Ability to effectively facilitate Support groups for residents

  • Have understanding Of Evidence-Based Practices, i.e. Wellness Self-Management, WRAP

Job Summary: The Case Aide (CA) will support residents in achieving their goals that promote recovery, wellness, and successful community living. CA will assist in the development of a culture of recovery and peer support, in which Project HOME alumni and residents are encouraged to build and strengthen relationships with their peers and maximize their use of community resources. The CA will work together with residential staff to support residents in achieving their goals, through the use of peer support, role modeling, connecting with and using community resources, assisting to build new competencies and other similar activities. Essential Duties and Responsibilities: * Reception * Be courteous and respectful of all residents, guests and community members * Assure that all guests sign the visitor's log * Be present at the front desk office at all times except when doing rounds and/or handling emergencies * Allow access to staff, residents, guests, and identified delivery people only * Check all exterior doors to make sure they are locked and secured. Check all doors and windows on 1st floor to make sure they are locked and secure * Make hourly rounds throughout the building * In emergency situations follow security procedures in manual and notify the appropriate personnel * Responsible for light housekeeping tasks and duties * Log & report any unusual activity * Participate in Shift Change with person taking over your shift to share information * Write a brief summary of your shift in the staff log book * Communication * Answer incoming calls, take accurate messages, and transfer calls as necessary * Check and respond to email regularly during shift * Assist with general administrative duties, such as faxing, copying, mailing, creating flyers etc. * Promote Recovery Community * Meet with residents and their support teams to assist residents in developing goal plans and recovery plans * Complete assessments and maintain ongoing progress notes for residents * Accompany residents to appointments when needed * Assist residents in acclimating to new housing/neighborhood * Assist residents in taking an active role in their own wellness by helping them implement new habits that promote independent living, health, and wellness. * Use one's own experience of recovery as a model for residents without directing residents' recovery experience. * Respectfully engage persons in recovery in a way that recognizes every person's recovery process is unique. Support them in identifying strengths, existing supports, and community resources. * Meet regularly with supervisor to provide updates and to seek feedback, participate in group supervision * Attend required trainings and continuing education courses as recommended by supervisor * Co-Facilitate Alumni meetings and help plan activities and outings for the Alumni program * Miscellaneous * A great deal of patience and a good sense of humor * Case Aid is responsible for light housekeeping and various additional clerical tasks * Log in deliveries and donations * Receive and sort mail and place in appropriate staff mail boxes * Complete a brief summary of events occurring during the shift in the log book Minimum Qualifications * GED or high school diploma required. * Ability to establish trusting relationship with peers and model community integration and independence. * Commitment to recovery, choice, empowerment, and the ability of people with serious mental illness or co-occurring disorders to find meaningful lives in the community. * Actively engaged in one's community (outside of one's residence/program), familiar and comfortable with accessing community resources. * Basic computer skills, including Microsoft Work and Microsoft Outlook. * Able to document work accurately and utilize a computer for email, calendar, and documentation. Preferred qualifications * Experience working in behavioral health preferred. * A self-identified current or former user of mental health or co-occurring services who can relate to others who are now using those services. * Peer Specialist Certification is strongly preferred. * Bilingual (Spanish) a plus. * Valid Driver's license strongly preferred. * Experience working with Harm Reduction preferred.
