Key Responsibilities:
Conduct and document monthly head-to-toe assessments of residential clients in their homes.
Maintain communication with agency management and the client's support team, reporting and documenting changes in health status and needs.
Ensure staff supervision, delegation, and specialized instruction are conducted and documented.
Complete monthly service logs, ensure authorized units are delivered and documented, and complete required training.
Conduct Abnormal Involuntary Movement Scale (AIMS) assessments for clients at risk of tardive dyskinesia.
Provide Level 1 Med Aide training to employees, if assigned.
Complete Health Risk Screening Tool (HRST) duties, if assigned, including initial evaluations, maintaining electronic medical records, and attending support plan meetings.
Assist in responding to HIPS, Mortality Reviews, and other Nursing Reviews by the Regional Office.
Adhere to PFH Policy and Procedure and act as an advocate for individuals served.
Education and/or Experience Qualifications:
Required License/Certification (one of the following):
Additional Qualifications:
Successful completion of background check, including criminal record, driving record, and abuse/neglect.
Valid driver's license, acceptable driving record, and current auto insurance.
Current CPR and First Aid certification.
Ability to operate calmly in crisis situations and relate positively to others.
Strong verbal and written communication skills.
Maintain a positive attitude when working with others.
