Residential LPN Care Coordinator, Full-Time, Day Shift-1.0

Licensed Practical Nurse/LPN, Full-Time, Day Shift, Milwaukee, WI 1.0 ($4,000 Sign on Bonus Available)

Community Care is hiring Licensed Practical Nurses/LPNs who are committed to teamwork and providing great care to become a part of our compassionate caregiving team. The Licensed Practical Nurse/LPN provides nursing care and services for our residents.This is a full-time opportunity on our day shift for Sacred Heart location at 1555 S. Layton Blvd, Milwaukee, WI 53215.

Licensed Practical Nurse/LPN Job Responsibilities:

• Work in collaboration with other nursing staff to coordinate and provide direction of care during weekday operations

• Serves as coordinator of care for scheduled weekend and holiday shifts

• Ensures care and service delivery are in conformance with regulations, policies, procedures, and standards, established by Community Care, the Program for all-inclusive Care for the Elderly and the State of Wisconsin.

Licensed Practical Nurse/LPN Job Requirements:

• Education: Diploma from accredited program in practical nursing required

• Licensure: A current Wisconsin Licensed Practical Nurse/LPN license in good standing

• Certification: Current CPR/AED certifications

Licensed Practical Nurse/LPN Job Schedule:

• Work Schedule hours: 6:30 a.m.

• 3 p.m. (40 hours per week)

• Every other weekend

• Every other holiday
