Job Description:

POSITION SUMMARY: Facilitate and work with consumers on a variety of activities of daily living (ADLs) needed for consumers to become more self-sufficient and move toward recovery. Additionally, you will facilitate group therapy, manage intakes/discharges, monitor medications, create a safe environment for the consumers, help coordinate treatment, and advocate for individuals with severe and persistent mental illness. Responsible for direct service care to consumers as part of a multi-disciplinary team.


  • Provide individual psychotherapy with consumers to provide support and insight.

  • Facilitate group therapy sessions within the house.

  • Coordinate clinical care with case management services and other services both inside and outside of MHCD.

  • Assess consumers throughout the workday, and provide appropriate interventions.

  • Encourage, support, and train consumers in activities of daily living (ADLs) - teaching consumers about hygiene, help make appointments, show how to do household duties/chores, such as cooking, cleaning, etc.

  • As part of a multi-disciplinary system, help facilitate the treatment and continuity of clinical care.

  • Complete intake and discharge tasks for consumers being place in the house as new residents or moving out of the house.

  • Maintain household safety in regard to a variety of household chores and activities, such as cooking, cleaning, and laundry.

  • Document all significant consumer interactions toward recovery and continuity of care.

  • Complete necessary clinical documentation, such as the daily log, consumer record, and medication records.

  • Respond to consumer and building emergencies, and facilitate appropriate interventions. In emergency situations, contact other residential staff, your supervisor, or other clinicians when needed.

  • Advocate for acceptance and integration of individuals with mental illness within the neighborhood and community in which they live.

  • Required to attend scheduled team meetings.

  • Perform other duties as assigned.

Job Requirements


Bachelor's Degree in social work, psychology, human services, or related field.


QMAP (Qualified Medication Administration Person)

CPR & First Aid

Physical and TB required. (must be completed prior to hire, and maintained during employment)


One year experience in mental health or human services preferred. Must be 21 yrs. of age to apply.

  • Knowledge and understanding of trauma-informed care principles/practices.




Computer, telephone, facsimile, and other machines found in a residential facility.


Requires sitting, standing, bending, lifting. Must be able to perform a variety of household tasks, climb stairs, move throughout the house, and quickly respond to emergencies. May require lifting 50 pounds.


Must meet qualifications under Chapter 7 of the Residential Care Facilities - Mentally Ill requirements. Shift work and/or weekend work is required. Certain programs may require a Class "S" driver's license.

Park Place POSITION SUMMARY: Provide direct mental health clinical assessments and treatment to individuals with mental illness. Work with others in a clinical team environment to help individuals move toward recovery. LCSW - $24.86/hr. - $29.81/hr. LPC/LMFT - $23.49/hr. - $28.17/hr. ESSENTIAL JOB FUNCTIONS: * Provide psychological evaluations/problem identification, crisis interventions, and individual, group, and/or family therapy to assigned clients. * Provide case management services to clients as needed or assigned. * Develop treatment plans; monitor treatment progress and follow-up at disposition times. * Assist clients with psychological development, social development, and rehabilitation. * Coordinate client's use of other MHCD programs and outside community resources. * Coordinate medication/medicinal needs of clients with psychiatrists, nurses, and other medical sources. * Maintain accurate and timely clinical records consistent with MHCD standards. * Participate in team/department meetings, in-services, and supervisory sessions as required. * Perform 27-10 (M-1) "mental health hold" evaluations. * Perform other duties as assigned. Job Requirements EDUCATION: Master's Degree in social work, psychology, or similar field. Current license as a LCSW/LPC/LMFT in the State of Colorado. EXPERIENCE: Minimum of three years experience in clinical social work. Experience working with moderate to severe and persistently mentally ill populations. SKILLS AND COMPETENCIES: * Knowledge of methods of psychotherapy. * Knowledge of community resources. * Ability to assess crisis situations and intervene appropriately. * Familiarity with the DSM V and diagnostic techniques. * Effective written and verbal communication skills. * Ability to work flexible and on-call hours which may be required. * Must have valid drivers license and good driving record. * Knowledge and understanding of trauma-informed care principles/practices. SUPERVISORY RESPONSIBILITIES: Assist with program supervision of students/volunteers as determined appropriate. TYPICAL PHYSICAL DEMANDS: Ability to sit, stand, bend and reach, drive, operate standard office equipment such as phone, computer, facsimile, copier/printer, and other business machines. Occasional lifting up to 40 lbs. TYPICAL MENTAL DEMANDS: Ability to give, receive and analyze information, formulate work plans, prepare written materials and articulate goals and action plans. Must understand people from all walks of life and be able to communicate effectively with them. WORKING CONDITIONS: May require occasional evening, weekend or on-call hours.
