Resident Services Manager - South Saint Paul

Walker Methodist River Heights is recruiting for an experienced Resident Services Manager to assist resident and family communication. Assistance may include physical and psycho-social needs, one-to-one visits, scheduling care conferences, resident or family meetings, providing resources, and connections to community resources and education. This position will work closely with all department directors to provide quality resident support services.

Minimum Qualifications:

• Bachelor's degree in Human Services-oriented field such as social work, psychology, gerontology or health care administration

• Two to five years experience in coordinating services to meet the physical, social and emotional needs of residents in a housing or long-term care setting

• Working knowledge of community services for older adults, to perform care conferences and discharge planning

• Commitment and skill to appropriately and professionally advocate for residents and families

• Ability to effectively collaborate with other disciplines, corporate staff and the community

• Demonstrate strong planning and problem-solving skills and writing ability

• Intermediate computer skills and electronic health record systems experience

• Ability to lead the team in Walker Methodist's Customer Service Principles
