Requirements to be a Resident Program Coordinator

Program Coordinators plan, develop and implement creative and exciting resident programs to meet the individual needs and interest of the residents within the community as well as through outside trips and events. Coordinators also endure specialty programs are scheduled, ensure daily, weekly and monthly calendar of events requirements are met as well as coordinating the transportation of residents to and from events outside the community. Associates Degree or equivalent with emphasis on recreation, health education or related field required.

Responsibilities The Resident Care Coordinator ensures delivery of quality assisted living services focused on maximizing residents' individual capabilities in accordance with all laws, regulations and company standards. ADMINISTRATIVE AND PLANNING * Plans, develops, organizes, implements and evaluates resident care goals with Director of Resident Care collaboration. * Compliant with budget in labor and expense control. * Determines appropriate staffing levels to meet resident needs 24/7 and ensures schedule is made. * Communicates resource needs to Resident Care Director. * Collaborates with Resident Care Director/Licensed Nurse regarding resident clinical needs and follow-up resolution. RESIDENT CARE FOCUS * Works with General Manager and Resident Care Director/Licensed Nurse to build relationships and strong communication with residents, families and associates. * Communicates regularly with family about resident's condition. * Evaluates residents for proper placement based on acuity levels. * Anticipates, plans and evaluates care of residents. * Assists new residents with transition adjustment to community. * Meets with residents regularly to encourage communication and gain understanding of their needs. * Makes daily rounds to review performance of resident care staff. * Assures Med Techs and Caregivers Follow Health Care Monitoring Procedure to report and monitor changes in Residents condition. * Assures ALL Med Techs and Caregivers read the 24-hour log at beginning of shift and Temporary Service Plans for reporting and monitoring change in resident condition * Monitors service records for all residents. * Ensures compliance with medical and contact information for all new residents. * Performs reviews resident chart notes for accurate, complete information. * Participates in family conferences. * Develops and monitors quality assurance plan. * Ensures communication with physicians and other healthcare professionals. * Reviews Medication Management compliance with Resident Care Director/Licensed Nurse. * Ensures compliance with Medication Pass Observation with a corrective action plan for deficiencies. * Conducts QA audits of medication records and med storage areas. * Assists with scheduling of ancillary services: podiatry, dentist, therapies, pharmacy consultation, etc. * Inventories, maintains and replenishes service supplies. * Ensures investigation and resolution of resident /family concerns and complaints/grievances. * Assists with discharge planning. * Arranges transportation for residents as needed. * Provides hands-on care as staffing needs require (and licenser allows). SUPERVISORY REQUIREMENTS * Assist Resident Care Director in interviews, hiring, training, evaluates and counsel's resident care team. * Supervision of the Caregivers and Med Techs along with Director of Resident Care * Schedules department hours appropriately. * Serves as an effective role model. * Approachable management style. * Practices firm but fair treatment of team. * Treats all associates respectfully. * Praises publicly and corrects privately. * Exercises appropriate independent judgment. * Remains calm in a crisis. COMMUNITY SPECIFIC FUNCTIONS * Foster a TEAM environment. * Create and maintain a Culture of Kindness within the team. * Assist Resident Care Director in "Department Best Practice for Resident Care". WORKING CONDITIONS * Works in office and throughout community. * Subject to frequent interruptions. * Works beyond normal working hours, on weekends, and in other positions temporarily as necessary. * Subject to falls, burns from equipment, odors, etc. throughout the working day. * Subject to hostile and emotionally upset residents and families. * Possible exposure to infectious waste, diseases, conditions, etc., including exposure to AIDS and Hepatitis B viruses. Qualifications EDUCATION AND EXPERIENCE * Must have 5 years' experience in senior services. * Must have 2 years' supervisory experience (senior services preferred). * Must have current CPR certification. * Must have current First Aid certification (if required by state). ESSENTIAL FUNCTIONS * Must be able to read, write, speak and understand the English language. * Must practice regular and predictable attendance. * Must follow company policies including harassment, code of conduct, complaint procedures, safety, sanitation, confidentiality, dress code and resident rights. * Must assist with evacuation of residents and supports residents in times of crisis (i.e., natural disaster). * Must monitor and report changes in resident condition. * Must be knowledgeable and competent to deliver quality nursing care to residents. * Must effectively assist with supervision of resident care team. * Must be able to communicate and function in an interdisciplinary team. * Must be able to concentrate and use reasoning skills and good judgment. * Must be able to sit, stand, bend, lift, push, pull, stoop, walk and move intermittently during working hours. * Must be able to lift 40 lbs floor to waist, lift 10 lbs waist to shoulder, lift and carry 40 lbs and push/pull 40 lbs.
