Community Assistance Network, Inc. (CAN) was founded in Baltimore County, Maryland in 1965 to eliminate the root causes and consequences of poverty. We presently serve clients in Baltimore County and surrounding jurisdictions and manage multiple programs providing a variety of services. CAN’s mission is to work in partnership with the community to develop, operate, and support programs that reduce vulnerability and promote personal growth, dignity, stability, and self-sufficiency among people in Baltimore County experiencing economic challenges.

Job Purpose:

Under the umbrella of CAN, the Homeless Shelters’ mission is to provide a safe, clean, and supportive community which helps stabilize homeless men, women, children, and families by promoting self-worth, providing linkages to community resources and encouraging personal responsibility. The Resident Intake Coordinator (RIC) is responsible for coordinating the 30/60/90-day shelter stays “triage approach;” assists in establishing shelter resident goals & developing resident plans for meeting objectives. The Resident Intake Coordinator reports directly to the Director of Shelter Operations.

The Resident Intake Coordinator (RIC) spearheads the shelters Triage Approach; manages the Daily Shelter Roster; conducts in-depth resident interviews and intakes, coordinates and sets team meetings with case managers, social workers and others; enters data into the Homeless Management Information System (HMIS).

Duties:

  • Spearheads the Triage Intake Coordination Approach.
  • Conducts and collects in-depth new resident intake assessments & inputs new data into Service
  • Point (web-based- information system) for shelter residents within 48 hours.

  • Enters intake survey information and all data into the Homeless Management Information
  • System (HMIS), specifically focusing on developing case management notes for triage coordination.

  • Works closely with Shelter Managers and Department of Health in assigning residents to
  • appropriate shelters for programming and services (convalescent care or routine shelter bed).

  • Assists case managers (including CAN Case Manager or DSS Case Manager; Rapid Rehousing
  • Specialists; Shelter Nurses; and/or outside case managers; any other mental health workers; other appropriate support from family, churches, hospitals etc.) with establishing the first Triage Team Meeting for residents within the first 15 days of the shelter stay.

  • Ensures 30/60/90-day team meetings are communicated and attended by residents and case
  • managers.

    Works closely with the Shelter Director & HMIS Coordinator in facilitating compliance to HMIS /Intake policies and procedures.

  • Accurately manages and distributes the Shelter Daily Roster.
  • Gathers HMIS numbers/data and past case management history
  • Provides copies of any identification, phone numbers and contact information for other resources to utilize in providing services.
  • Works closely with the Director of Homeless Services and the
  • HMIS Coordinator in facilitating compliance to HMIS/Intake policies and procedures.

    Minimum Skills/Qualifications:

  • Bachelor’s Degree preferred. Experience working with homeless or at-risk population is
  • preferred. Human service experience may be substituted for college degree.

  • Strong administrative and/or leadership experience.
  • Effective interpersonal, verbal and written communication skills.
  • Ability to develop and plan programs.
  • Knowledge of community services.
  • Computer literacy and proficient in Microsoft Office (specifically Word and Excel); efficient in
  • data entries into HMIS.

    • Ability to work well under pressure.

    • Ability to understand programming and services documents.

    • Strong organizational skills.

    • Valid State of Maryland driver’s license and transportation required.
