POSITION SUMMARY

As directed by health services leadership, assist in ensuring resident care needs are met by health and supportive services provided to residents at the Assisted Living Community.

Coordinate and oversee care staff work schedules. Provide resident care and services. Assist health services leadership in the oversight of health services procedures and systems.

ESSENTIAL DUTIES AND RESPONSIBILITIES Include the following. Other duties may be assigned.

  • Assist in the development and implementation of Cooperative Service Plans.

  • Ensure that Cooperative Service Plans are completed and updated in a timely manner.

  • Perform various resident evaluations as assigned by Health Services Leadership.

  • Coordinate and adjust resident specific service schedules.

  • Assist in the development of staff work schedules.

  • Provide direction and oversight to healthcare staff and assist in staff education and development as assigned.

  • Responsible for coordinating services, interviewing and hiring, shift coverage and staff scheduling.

  • Assist by encouraging and/or reminding residents to participate in activities. Promote activities enthusiastically to pique residents' participation in activities. Facilitate activities outside of the facility when requested.

  • Observation and oversight of the Documentation and Communication Policy and Procedure and Medication Room systems.

  • Coordinate information pertaining to changes in Residents' functional status, personal needs, appointments, outside services, etc. Coordinate transportation services and resident health services appointments for offsite visits.

  • Provide resident care and services covering shifts as needed or assigned by the health services leadership. Work as Medication Aide and/or Personal care Attendant at least one shift per week.

    Report concerns about residents to supervisor and co-workers. Practice safety working around and with equipment, chemicals, tools and utensils (infection control; universal precautions).

  • Respond to resident emergencies; provide first-aid assistance and arrange for appropriate medical attention/follow-up.

  • Health Services supply ordering.

  • Provide on-call support for staff questions and staffing coordination.

COMPANY WIDE RESPONSIBILITIES

Supports a dignified and caring atmosphere with residents, residents' families, visitors and staff. Protects privacy and confidentiality of information pertaining to residents, employees, the residence, and company information and records.

Maintains a safe and secure working environment and practices safe working habits. Complies with the Prestige policies. Maintains neat appearance, good personal hygiene, and appropriate attire. Assists with special projects as assigned and performs other related duties.

SUPERVISORY RESPONSIBILITIES

Assists health services leadership in the oversight of personal care assistants.

QUALIFICATIONS

High school diploma or GED, with reading, comprehension and writing ability at 12th grade level or higher. Clean driving record (verified by MVR) and willingness to drive the company vehicle to transport Residents.

Ability to work weekends and holidays as scheduled. Must be able to perform essential job duties with or without reasonable accommodation.

EDUCATION and/or EXPERIENCE

Training/Experience as a nursing assistant or home-health aide helpful. Certified medication training preferred.

Skills in nurturing and organizing similar to those in a domestic household. Experience or interest in working with older adults.

Essential Functions: * Provides direct resident care while promoting resident dignity, independence and choice, regularly observes residents' physical and mental conditions and reports changes to the Health Services Director, Expressions Coordinator, or Executive Director. * Assists with or performs ADL's, takes vital signs and weights and accurately documents care services provided and observations on communication log and incident reports. Reports and documents all incidents and accidents immediately. * Assists residents to and from dining room and activities. * Performs First Aid and initiates emergency response procedures as required. * Greets residents, visitors, and staff courteously and answers phones when assigned. * Follows and supports community fire safety and infection control practices and promptly reports any unsafe conditions or equipment to immediate supervisor. * Serves meals in the dining room, assisting residents at meal time, provides resident tray delivery, and assists with dining room and kitchen clean up and maintenance. * Assists with tours, move-ins, move-outs, and transfers and coordinates personal care service with co-workers assisting with lifting and responds to assistance pages promptly. * Assists with apartment housekeeping services and residents laundry. * Attends and participates in training, in-services, and staff meetings. * Performs other related duties as assigned.
