GENERAL SUMMARY:

The Resident Care Coordinator (RCC) facilitates the personal well-being and attends to the non-medical needs of the Assisted Living resident in accordance with the governing federal, state, local, and facility standards, guidelines, and regulations.

ESSENTIAL JOB DUTIES

  • Responsible for complete case management services of Residents in Assigned Area; including, but not limited to, pre-move-in and move-in services, providing input for ongoing assessments, move-out or transfer planning, and special and/or urgent meetings.

  • Provides ongoing chart maintenance and audits for compliance with all applicable rules and regulations. Charts pertinent information on residents.

  • Assures new residents receive a welcome and an introduction to staff and other residents.

  • Explains evacuation, disaster, and fire drill procedures to new residents and/or their responsible party and revisits the information as necessary. Participates in drills and drill evacuations.

  • Initiates and updates the resident's profile and face sheets regarding changes in demographics or status.

  • Conducts Evaluation and Dietary Profile prior to Service Plan updates and Care Conferences according to the Service Plan schedule in coordination with the Assisted Living/Memory Care Administrator and/or applicable staff.

  • Conducts Level of Care evaluations during Service Plan Reviews and as resident needs change in coordination with the Assisted Living/Memory Care Administrator, RN, and/or applicable staff.

  • Prepares ongoing Service Plans for residents in coordination with the Assisted Living/Memory Care Administrator and/or applicable staff.

  • Sends notification of Care Conference meetings to family members and coordinates monthly Care Conference meetings.

  • Conducts pre-admission screening of prospective new residents when assigned by supervisor.

  • Observes residents' health status, takes actions to address concerns within scope of practice, and reports all significant changes, reactions to medications and treatment or significant incidences to the Health Services Manager (LPN), Health Services Director, Administrator, Executive Director, physician, or pharmacist (if applicable) in a timely manner.

  • Performs daily, weekly, and monthly safety and infection control checks of the home and grounds, assisting the Administrator and the Health Services Director for resident health and safety purposes.

  • Participates in the maintenance of resident records including review of documentation performed by RSA's, setting up new residents and thinning records appropriately.

  • Participates in department quality improvement activities, to include completion of quality assurance surveys as assigned by Administrator and/or Health Services Director.

  • Accounts for resident's funding sources, changes, and ensures preparation of proper documentation.

  • Serves as the main liaison for Assisted Living operations and residents, family members, staff, and volunteers.

  • Assists residents with transportation arrangements as needed.

  • Assists residents and families with shopping needs by monitoring supplies and ordering needed items as applicable.

  • Responds to monthly census confirmation from the Business Office.

  • Prepares monthly financial reporting, which includes guest meals, medical supply, tray service, health shakes and admission fees/charges as applicable, and any necessary adjustments to the Business Office.

  • Reports when resident is out of the facility to Business Office.

  • Assists with special activities for residents and community involvement.

  • Provides additional/supplemental coverage on the floor when needed.

  • Supports staff with personal care duties as needed.

  • Performs ongoing evaluation of resident physical and psycho-social needs and coordinates with nurse to assure quality proactive care as allowed by state regulation.

  • Participates in staff training.

  • Assists in Abuse Investigation process by processing through Incident Reports and closing out investigations in a timely manner.

  • Supports & assists the administrator to address and process complaints.

  • Assists administrator as requested on special projects.

  • Works collaboratively with Administrator, HSM's, Staffing Coordinator and the Memory Care Administrator.

  • Shares on call duties as required.

  • Other duties and projects as assigned.

Qualifications

QUALIFICATIONS:

  • Minimum of 1 years of experience in senior care.

  • Two-year degree in Social Services or related field preferred.

  • Experience serving seniors with Alzheimer's and/or dementia.

PHYSICAL REQUIREMENTS/WORKING CONDITIONS: Normally works in well lighted, comfortable surroundings. Must be able to concentrate with frequent interruptions. Must be able to sit/ stand for up to ninety percent (90%) of the workday. Occasional walking; frequent bending, reaching and stooping. Must be able to tour through the entire community up to one mile; occasionally pushing a wheelchair. Must be able to lift and transfer up to fifty (50) lbs. independently. May be exposed to communicable diseases including the HIV and HBV viruses, unpleasant odors, and combative behavior.
