General Purpose:

p, As part of a high-functioning leadership and care team, conduct and coordinate the development and completion of the resident assessment in accordance with the rules and regulations of State and federal government and the policies and goals of the facility. Reports to the Director of Nursing.

Essential Job Functions:

Our employees are expected to promote an atmosphere of teamwork with other employees and hospitality and comfort for its residents. Therefore, the following list of duties is not all-inclusive:

  • Maintain MDS calendars of all residents, Medicare, MaineCare, Managed care and private pay. Ensure that all components are completed based on requirements.

  • Communicate MDS due date so that interdisciplinary team members complete their portions appropriately and on time.

  • Instruct all disciplines in proper procedures, completion of MDS items, and CAAs, Care Mix payment and Quality Indicators.

  • Completion of the MDS by the RAI Coordinator or licensed designee

  • Oversee and/or input MDS into computer system

  • Oversee and/or complete CAAs other disciplines as designated sections as appropriate, ensure correct dates and signatures upon completion

  • Oversee and/or complete care plans for all residents process according to standard of nursing practice, including nursing restorative plans

  • Review all care plans on a regular basis for accuracy, appropriateness, and case mix validity

  • Assess/Oversee assessment of all residents through the RAI process and ensure coding accuracy, capturing needs and level of assistance, in accordance with current rules, regulations, and guidelines

  • Establish documentation approaches and instruct nursing and ancillary staff to document properly

  • Analyze case mix reports regularly to determine scheduling, assessment, care planning, and documentation practices that manages case mix process systematically in the facility

  • Provide weekly case mix report to Administrator, DON, and others per facility practice

  • Oversee and/or submit/transmit electronically all RAI records in a timely manner

  • Verify all submitted/transmitted records for acceptance and validity of RUG groups

  • Correct any outstanding errors as necessary

  • Verify facility roster as necessary, make corrections to ensure proper payment via rate lette

  • Verify on a monthly basis, the Home Office Billing Departments PPS Report

  • Maintain accurate/current MCR calendars/boards

  • Download QI reports upon request and/or instruct others on procedure to download reports

  • Provide analysis of QI trends, outstanding and new problems individual and facility wide.

  • Provide report to Administrator DON, and other staff members upon request.

  • Download QM reports upon request and/or instruct others on procedure to download report

  • Provide analysis to Administrator, DON and other staff members upon request

  • Educate staff on meaning/use of QMs

  • Provide yearly calendar to their supervisor stating education plans, regarding all required documentation

  • Keep all staff updated on latest changes to rules and regulations of State and Federal government related to the RAI process

  • Educate on case mix report indicators, i.e., staffing allocation based on level of care

  • Educate staff on utilizing Qls to improve quality of care

Minimum Qualifications:

  • Current, active, Maine RN licensure

  • Strong nursing clinical assessment, care planning, and delivery of care skills, with a long term care population

Skills and Abilities:

  • Working knowledge:

  • Geriatric health care needs

  • Federal and State regulations pertaining to long term care and the resident assessment process

  • Medicare and MaineCare regulations, as well as the application of the State Operations Manual, and Principles of Reimbursement

  • MDS manual and updates; Quality Indicator System

  • Strong computer literacy. Experience with and ability to document within electronic medical records.

  • Work and communicate tactfully with residents, staff; administration, families, physicians, ancillary services, allied health care professionals, surveyors, regulators, other business contacts, the public and the community
